The alteration of the structure of mortality of endocrine system diseases, nutrition disorders and metabolic disturbances in individuals of able-bodied age and older in conditions of pandemic

Abstract
The elderly age and endocrine diseases are among death risk factors at contamination with new coronavirus infection. To reply the question of how the influence of these risk factors is summing up, differences were determined concerning patterns of changes in structure of mortality of endocrine system diseases, nutrition disorders and metabolic disturbances in individuals of able-bodied age and older against the background of COVID-19 pandemic.on the basis of information from the Moscow database of deaths of endocrine system diseases, nutrition disorders and metabolic disturbances, the changes in contribution of individual diseases to the death causes structure in 2019-2021 were analyzed. The groups of individuals of able-bodied age and older were compared.It was established that in both groups rate of indicating SARS-CoV-2 virus infection as concomitant disease decreased while rate of indicating concomitant diseases at death of COVID-19 increased. The group differences in changes of structure of death causes were established in 2021. The percentage of undetermined forms of diabetes and obesity in structure of death causes increased in the elderly, while in individuals of able-bodied age increased. The percentage of obesity as concomitant disease of death of COVID-19 in individuals of able-bodied age increased and did no change were detected in the elderly group. The input of poly-glandular dysfunction as consequence of old infection into mortality is five times higher among individuals of able-bodied age.On the basis of received results, assumption was made SARS-CoV-2 virus contamination ruinously affects development of pathological process under endocrine diseases regardless of age, while age affects spreading of endocrine diseases and degree of resistance to development of infectious process directly. The differences in patterns of changes of structure of death causes of population of able-bodied age and older are associated with low quality of diagnostics of death causes in individuals of elder age groups.

The transformation of medical statistic during pandemic of new coronavirus infection (COVID-19)

Abstract
The pandemic of new coronavirus infection (COVID-19) directly effected medical statistics service. The amendments to the Federal Law “On the Official Statistical Accounting and the System of State Statistics in the Russian Federation” adopted in December 2020, regulated the provision of primary statistical data according forms of Federal and industrial statistical observation in the format of electronic document signed with electronic signature. This required the development of system of collecting and processing statistical data at the Federal level applying new technological solutions.The purpose of the study. To analyze the changes in the system of collecting and processing annual reporting on medical statistics for 2020 during the pandemic of new coronavirus infection COVID-19.The analysis was made concerning both normative legal base regulating implementation of information systems and system of receiving annual reports for current and previous years. Also content analysis was applied and materials distributed Internet were used.In conditions of new coronavirus infection (COVID-19) pandemic, instead of classical system of informational interaction at face-to-face coordination of annual report data, in extremely short terms new model of informational interaction of remote coordination and processing of annual report data was developed. The updated technological scheme was applied that included data transmission, remote coordination in VKS format, informing thriugh Telegram-channels and signing finalized forms with enhanced electronic digital signature.The complicated epidemiological situation regarding morbidity of new coronavirus infection (COVID-19) and as well as adoption of amendments in Federal legislation regarding provision of statistical observation forms in format of electronic document signed with electronic signature, required revision of format of statistical reports reception in 2020. The application of developed technology of collecting and processing annual reports data on medical statistics in online format permitted to dispense with both provision of hard copies versions of forms and reports and business trips of specialists from the subjects of the Russian Federation to Moscow that reduced expenses of the subjects of the Russian Federation when submitting annual reports. The developed mechanism of signing with unqualified electronic digital signature permitted to control data integrity. The applied mechanism for signing finalized concerted forms with enhanced electronic digital signature of public authority of the subject of the Russian Federation in the field of health protection ensured juridical significance of document provided. The establishment of base for further modernization of system of collecting statistical information from primary data, including subsequent implementation of structured electronic medical documents is proposed.

The social capital of organization in conditions of increasing social economic risks: a publications review

Abstract
The article considers essential theoretical methodological approaches of studying problematics of social capital presented in corresponding modern publications. The key results of conducted empirical studies were analyzed too. It is noted that in conditions of spread of COVID-19 pandemic, occurs increasing of risk of “social distancing” that affects intensification of psychological anxiety feeling and decreasing of level of interpersonal and institutional trust. The presented results testify that topic of social capital becomes one of the most important in the study of processes of pandemic course. The conducted studies permitted to identify relationships between various components of social capital and scale of pandemic spread, forms of population's counteraction to pandemic. The review of scientific publications on problematics of social capital and functioning of organizations in conditions of intensification of risks of pandemic spread and developing crisis processes in economics also permits to establish particular trends in respect of developing corresponding strategies of adaption to new reality.

The morbidity with temporary disability and its regional characteristics in the Russian Federation in 2000-2018

Abstract
The article presents the results of analysis of dynamics of morbidity of working population with temporary disability in the Russian Federation in 2000-2018 according its causes, assessment of gender and age differences and in the context of Federal Okrugs and subjects The Russian Federation. The significant changes in rate and duration of temporary disability indices on National and Regional levels are considered. The absence of differences in structure of causes and average duration of morbidity with temporary disability is analyzed. The most “damaging” gender and age groups of working population in cases and days are determined, that determines need in development of target efficient measures of preventing and reducing temporary disability in Russia.

The development of health preservation technologies: from individual to family approach

Abstract
The actuality of considered problem is in the fact that many medical social problems of preserving and improving population health can be resolved by the family and with its support. The purpose of the study is to substantiate scientifically priority of family approach in development of National health preservation system in actual conditions. Materials and methods. The national and foreign electronic databases were investigated to find publications concerning transformation of matrimonial relations, effect of family on health and need in health preservation technologies at each stage of its development. Results. It is demonstrated that implementation of family-oriented technologies of health preservation based on knowledge of trends in family health, matrimonial relations and demographic shifts in its structure is one of the priority directions of preserving and strengthening population health. Moreover, in modern socioeconomic conditions, the categories of families that are in special need of medical social protection and health preservation such as socially problematic families, families where violence is practiced, families of the unemployed and migrants are shown up. Hence, one of the priority scientific tasks is to substantiate conceptual model of health preservation technologies for various types of families. Conclusion. The family and its health should become priority object of medical observation and research studies from the standpoint of public health and health care.

The adolescents, Internet and drug abuse

Abstract
According to results of research study, in 2016-2017, the number of Internet users among schoolchildren was 96% of boys and 97% of girls and at that 44% of boys and 60% of girls spend 3 hours or more on the Internet daily. Quite often excessive infatuation for Internet is combined with psychoactive substances abuse.The purpose of the study is to identify features of abuse of drug and other toxicomanic substances among adolescents who used the Internet continuously on daily basis. The study was carried out within the framework of all-Russian monitoring of consumption of psychoactive substances among rural adolescents-schoolchildren aged 15-17 years in 2016-2017. The multistage sampling included 18 222 rural adolescents (8 208 boys and 1 014 girls). The statistical obtained data was processed using the AnkProc software (“Risk factors”). To evaluate statistical significance of differences in indices the Pearson criterion 2 was applied.The comparative analysis of data concerning rural adolescents continuously using and not using the Internet on a daily basis established that level of experimentation and use of drug and toxicomanic substances among adolescents continuously using the Internet on a daily basis is statistically significantly higher than among adolescents continuously not using the Internet on a daily basis. The intensity of involvement of adolescents continuously using the Internet on a daily basis in use of drug and toxicomanic substances is statistically significantly higher than among adolescents continuously not using the Internet on a daily basis. No statistically significant differences were established in these groups based on the reasons for the introduction and use of drug and toxicomanic substances. In terms of awareness of dangers of use of drug and toxicomanic substances, adolescents continuously using the Internet on a daily basis are less aware as compared with adolescents continuously not using the Internet on a daily basis.Thus, despite daily long-term occupation and absorption with Internet, adolescents from this group not only experiment, but also use drug substances statistically significantly more than their peers from the group who do not use continuously the Internet on daily basis.

The chronic cardiac insufficiency as a cause of death: the problems of statistical service and evaluation

Abstract
The purpose of the study is to analyze the application of I50.9 code as initial cause of death (PBC) in various subjects of the Russian Federation in 2013-2019.The Rosstat data (Annual forms C41 and C51) based on the “Brief Nomenclature of Causes of Death of Rosstat” (KNPSR) in 2013-2019. The standardized mortality rates (SPS) were determined using corresponding software (state registration number 216661114), coefficient of variation (Cv) and the max/min ratio.In 2013-2019, the regional average value of the SCR from cardiac failure (code I50.9) decreased up to 4 times (from 6.3 ± 10.3 to 1.5 ± 3.5; p <0.0001), but Сv of regional SCR increased by almost 1.5 times (162% and 230%). The percentage of regions where cardiac failure was not indicated as PPP, increased from 8% in 2013 to 19% in 2019. However, only in the Yaroslavl Oblast and the Republic of Buryatia no case of indicating I50.9 code as PPP was registered. In five regions, less than 10 cases were registered in 7 years, in 3 regions an unstable tendency to increase in SDR from HF was established. In 36 regions, more significant variability of RMS from year to year was recorded. From our point of view, the indices are affected in parallel by teo processes: changes in mortality rates associated with chronic heart failure and changes in mortality rates due to approaches to filling out the MSS and coding PPP.To determine the contribution of cardiac failure into population mortality, it is necessary to separate concepts “chronic cardiac failure syndrome” and “cardiac failure as death mechanism” and to introduce uniform rules of indicating chronic cardiac failure syndrome in the MSS.

The orphan preparations: international approaches and national regulation

Abstract
The medication support of patients with rare (orphan) diseases is an actual issue, since it requires significant financial costs due to high price of medications. The accessibility of these medications to such patients should be regulated by state incentives for development of orphan medications with following taking out to the market, by procedure of composing list of medications for treatment of orphan diseases and determination of financial support sources.The article considers aspects of regulation of issues related to orphan diseases and orphan medications in the Russian Federation and Europe. On the basis of regulated list of orphan diseases in Russia, the results of comparative analysis of list of orphan medications in Russia and list published at website of the European Union are presented. On the basis of data obtained, it was established that many medications for treatment of rare diseases registered in Europe are not available in Russia. Also, many orphan drugs in Europe do not have this status in Russia. At the same time, there are no state incentives for development of orphan medications in the Russian Federation, and there are only some benefits in registration procedure of such drugs. The analysis demonstrated availability of development and taking out to the Russian pharmaceutical market large list of orphan drugs from the European listing for National corresponding manufacturers.

The social and medical problems of childhood psychiatry

Abstract
The purpose of the study is to to identify key social and medical problems of childhood psychiatry by analysis of statistical and clinical data of Moscow health care institutions. It is established that actual problems are related to increasing of incidence of morbidity of mental disorders in children, their late detection, absence or unavailability of diagnostic techniques with high evidentiary effect, inability to use modern achievements of psychopharmacotherapy, close relationship of mental disorders with social factors, as well as with high stigmatization of mental health sphere. It is necessary to intensify research in studying etiopathogenetic factors, elaborating strategies of mental disorders prevention in childhood, clinical trials targeted to expand age range of application of medications and to icrease activities on destigmatization and psychoeducation in society.

The dynamics of alterations and prognostic models of levels of morbidity of urban and rural population of the subject of the Russian Federation

Abstract
The article presents data of analytical study of morbidity indices of urban and rural population of the Republic of Tatarstan. The dynamics of population morbidity rate in the Republic of Tatarstan is characterized by steady trend of decreasing of indicator and is described by polynomial trend in both absolute and relative indices. The highest rates of increase in population morbidity were mainly observed until 2010. The maximal periods of decreasing or slowdown of rate increasing were characteristic in 2011-2019. In the structure of population morbidity, the highest percentage fell on diseases of respiratory and circulatory systems, injuries, poisoning and some other consequences of external causes impact that was typical both for the Republic of Tatarstan in whole and its rural territories. In general, morbidity rate of urban population was higher than that of rural population. This difference was statistically significant, but growth rate of number of cases in rural territories outstripped growth rate of number of cases in urban territories and was higher than in the Republic of Tatarstan in whole, but was not statistically significant. The medium-term prognosis of dynamics of population morbidity rate in the Republic of Tatarstan in whole demonstrated that, all other things being equal and taking into account current trends, decrease in number of registered patients with diagnosis established for the first time in life can be expected to 2874.2 thousand of population by 2024 (that is less as compared with 2019 level by 242.4 thousand of population, or by 7.8%) and number of registered patients with diagnosis established for the first time in life to 70217.8 per 100 thousand of population (which is less as compared with 2019 level by 9680.8 per 100 thousand of population, or by 12.1%).

The ensuring and protection of rights of persons suffering of mental disorders: analysis of industrial legislation

Abstract
The current world-wide trend of increasing of mental diseases morbidity requires active development of corresponding enhanced public legal measures. On one hand, it is necessary to ensure implementation of all civil rights of mentally ill persons. On the other hand, it is necessary to ensure safety of society, protecting it as much as possible from illegal behavior of mentally ill people. According to the WHO data, number of people with mental disorders is on average about 450 million people and this figure is constantly increasing. According to the Constitution of the Russian Federation, the state guarantees equality of human rights and freedoms of person and c itizen regardless of any circumstances. The protection of rights and legitimate interests of mentally ill people causes a number of difficulties, therefore they are vulnerable to community. The article identifies principal problems existing in this field and suggests possible means of eliminating them by introducing changes into current National legislation on mental care.

The organizational culture of medical organizations as a factor of carrier orientations of medical personnel

Abstract
The problems of professional advancement of medical personnel, their professional formation and professional career have enduring importance in connection with development and improvement of medical care system in any society. The purpose of the study is to investigate characteristics of influence of type of organizational culture of medical organizations on career orientations of medical personnel. The study covered sampling of 7 364 employees of medical organizations. The complex of psycho-diagnostic techniques was applied that permitted to examine prevailing type of organizational culture of medical organizations (The R. Harrison questionnaire adapted by S. A. Lipatov), preferences of medical personnel in building up their professional career (E. Schein questionnaire “Carrier Anchors”), and their concepts about competitive medical worker (semantic differential - 16 opposite personality characteristics). The statistical methods included standard procedures of primary mathematical processing of empirical data (frequency and percentage distribution, calculation of mean group indices, standard deviation and dispersions), comparative analysis using Student's test (correctness of criterion application was checked by the Libya test), Spearman correlation analysis.It is established that under developed organizational culture of medical organization, its orientations towards task and personality, with team cohesion and high level of value-orientated unity of their members, medical workers have consistency of concepts concerning career and a unity of career orientations targeted to professional competence and service to other people that determines quality of medical care.

The characteristics of life-style of medical students

Abstract
The article considers the results of study of characteristics of lifestyle and behavioral risk factors of students of the medical university. Materials and methods. The object of the study was students of medical university. The survey was implemented in remote mode using the Google Forms. The relative values and their confidence intervals were calculated using the Wilson method. MS Excel 2019 and IBM SPSS Statistics 19 software were used to statistical process the research results.Results. It is established that only every tenth student observed recommended duration of night sleep. The youths were about 1.7-4.6 times more likely to violate rules of personal hygiene as compared with girls. The unreasonable diet and low motor activity were more typical for girls. The prevalence of active tobacco smoking and nicotine use in dosage more than 0.1 mg/day among youths was 1.4 and 2.0 times higher than among girls, respectively (p<0.05). More than a half of respondents consume alcoholic beverages daily. At that, 44.12% of girls do not maintain sexual life, while among youths there were 1.5 times less of such individuals. The condoms during sexual contacts are obligatory used by 52.18% of youths that is 1.3 times more often than among girls. Almost three-quarters of medical students have insufficient alertness about their health and are characterized by irresponsible medical behavior. Conclusions. The gender differences in prevalence of lifestyle risk factors among medical students were revealed.

The morbidity of medical personnel of mobile teams of emergency medical care

Abstract
The medical personnel of mobile teams of emergency medical care are exposed to factors of working environment that affect health state and results in development of occupational diseases. The purpose of study is to analyze morbidity of medical personnel of mobile teams of emergency medical care according data of temporary disability and results of medical check-ups. The analysis was made according information from certificates of incapacity for work and the final acts of periodic medical examinations for five-year period. The study established significant increase of rate and duration of primary and general morbidity with increase of age and length of service of workers, higher susceptibility to diseases of physicians as compared with paramedics and employees of pediatric teams and specialists of other clinical profiles. In the structure of morbidity according to results of medical examinations, diseases of the eye and its accessory apparatus, the circulatory system and the musculo-skeletal system prevailed.

The characteristics of repeated medical social expertise of adult population of the Russian Federation in 2018-2020

Abstract
The medical social expertise is considered as recognition of a person as the disabled and specification, in accordance with established procedure, of needs of examined person in social protection measures, including rehabilitation, based on evaluation of disabilities caused by persistent dysfunction of organism. The main normative legal acts governing activities of institutions of medical social expertise are the Federal Law of November 24, 1995 “On social protection of disabled people in the Russian Federation”, the Decree of the Government of the Russian Federation of February 20, 2006 № 95 “On the procedure and conditions for recognition disabled person”. Because of COVID-19 pandemic conditions, the normative legal base has undergone a number of changes. In particular, on 09.04. 2020, the Resolution of the Government of the Russian Federation № 467 “On the temporary procedure for recognizing a person as disabled” came into force. The actual article considers the features of this temporarily order related to the repeated medical and social examination and its results as of 2020. The qualitative analysis of permitted to get patterns of increasing and decreasing (improvement or deterioration) of particular statistical indices.

The condition and tendencies of cervical carcinoma diagnostic in the Russian Federation

Abstract
The elimination of cervical cancer is one of the leading international public health problems that is associated with corresponding high morbidity and mortality among women of able-bodied and reproductive age. In the Russian Federation, in 2018, in more than 17.5 thousand women cervix malignant neoplasms were diagnosed and more than 6 thousand of them died. Cervical cancer is preventable disease that can be eliminated if the targets of the WHO Global Strategy to Eliminate Cervical Cancer as Public Health Issue will be achieved. The purpose of the study is to analyze indices of timely cervical cancer diagnostics in the Russian Federation and in its subjects in 2007-2018. Despite favorable trend of increasing of active detection of cervical cancer, this indicator (41.8%) continues to be low, both in the country in whole and in most of the subjects of the Russian Federation. The cervical cancer in situ stage is detected only in 25.3 cases per 100 cases of cervix cancer and stages I and II of disease are detected in 66.1% of cases. More than a third of all cases of cervical cancer (32.6%) are detected at III-IV stages of oncologic process. The percentage of patients died within the first year after cervical cancer was diagnosed decreased significantly from 19.0% in 2007 to 13.8% in 2018.

The methodology of prognostication of severe exacerbation of bronchial asthma and asthmatic status

Abstract
The modes of predicting chronic non-infectious diseases in clinical practice remain actual issues for general practitioner. The methodology of predicting severe exacerbation of bronchial asthma and asthmatic status during therapeutic care rendering in out-patient conditions is proposed. The methodology comprises sequential assessment in patient availability of predictors of severe exacerbation of bronchial asthma and asthmatic status with further calculation of indices of information values of each of them separately and as well as accumulated information value to evaluate their cumulative effect.

The organizational principles of medical support of ceremonies of opening and closing international sport competitions (as exemplified by the XXVII Summer World Student Games of 2013 in Kazan)

Abstract
The results of implementation of developed approaches to medical support of ceremonies of opening and closing international sport competitions are demonstrated by the example of the XXVII Summer World Student Games in Kazan in 2013. The ceremonies took place at the venue with capacity of 45,000 spectators. The number of requests for medical assistance during the opening and closing ceremonies was 5.1 and 2.5 per 1000 spectators and accredited participants, respectively. Six individuals were evacuated to hospitals (0.1 cases per 1000 spectators and accredited event participants). The experience of organization of medical support of opening and closing ceremonies of international sport competitions is of practical significance in the framework of preparation and holding of international events in Russia.

Abstract
The analysis of level and structure of general and primary morbidity according to medical care appealability of Moscow population during implementation of the State program “The Development of the City of Moscow Health Care (Metropolitan Health Care) in 2012-2020” was carried out according to the data from collections of main indices of functioning of medical organizations subordinated to the Moscow Health Care Department and health indices of Moscow population. The decreasing of both primary and general morbidity indices is established. At that, increasing of the life expectancy indices was marked. At the same time, population morbidity indices according to medical care appealability are not sufficiently reliable in assessing degree of achievement of results of any measures of improving population health due to their dependence on multitude of factors.

The mortality of population of the Russian Federation resulted due to alcohol toxic effect in 2010-2019

Abstract
The purpose of the study is to investigate dynamics of mortality rate of the population of the Russian Federation as a result of toxic effects of alcohol in 2010-2019.The statistical materials of the Federal State Statistics Service of the Russian Federation were used. The analysis of mortality of population of the Russian Federation because of toxic effect of alcohol was applied. The comparative analysis of population mortality of alcohol intoxication in the Far Eastern, Privolzhsky, Northern Caucasian, Northwestern, Siberian, Ural, Central and Southern Federal Okrugs was implemented. The statistical processing of initial data was implemented using common techniques of calculating extensive and intensive indices, indicator average error and evaluation of differences reliability.It is established that in the Russian Federation, in 2010-2019 population death rate as a result of toxic effect of alcohol decreased by 1.9 times. The decrease of mortality occurred in all Federal Okrugs. The most rapid mortality decreasing occurred in the Far Eastern, Siberian and Southern Federal Okrugs. By the end of studied period, highest mortality rate due to toxic effects of alcohol were registered in the Siberian and Ural Federal Okrugs and the lowest mortality rates were observed in the North Caucasus and Southern Federal Okrugs. This fact testifies presence of peculiarities in formation of toxicological situation at the regional level. The study of level and dynamics of mortality as a result of toxic effects of alcohol will permit to evaluate toxicological situation and to determine strategic directions of preventive activities in the field of chemical safety at the regional level.

The article presents the original system of indices for comprehensive assessment of level of intersectorial collaboration in palliative medical care support of population. The system was developed on the basis of and considering requirements of normative legal acts of the Russian Federation. The proposed indices can be applied in analysis of functioning and subsequent optimization of medical institutions network and social service institutions in the subjects of the Russian Federation, including involvement of non-profit organizations and public associations.

The sociological aspects of organization of operation of district psychiatrists in modern conditions

Abstract
The article presents results of sociological survey of district psychiatrists that revealed actual difficulties and shortcomings in organization of medical care for mental patients. The problem of personnel high workload, inopportuneness and inadequate completeness of necessary medical care of mental patients are explained by a number of difficulties in organization of functioning of psychoneurological organizations. They included not only shortage of personnel and unsatisfactory material technical support of mental institutions, but also incompleteness of normative legal support mental care organization. The physicians identified the following priority targets to be implemented in practice will eliminate a number of actual problems in district psychiatrist work: organization of procedure of interdepartmental interaction of various services (GUFSIN, the Ministry of Internal Affairs, social protection agencies, passport office), development and approval of procedure of dispensary observation of mental patients, approval of procedure of interaction with territorial polyclinics and full implementation of medical care standards for patients with mental disorders. The administration of neuropsychiatric organizations from position of systemic approach is a priority direction of improving quality of medical care for mental patients.

The evaluation of compliance of medical activity to established requirements: organizational and technological support

Abstract
The ongoing process of reforming national health care system determines associated search for systems of adequate decision-making based on comprehensive and objective analysis of activities of sector objects. The results of many years research demonstrate that one of the most effective such systems is system of assessing compliance of medical activities with established requirements. It is a methodology that permits to implement systematic comprehensive analysis of health care objects and make necessary decisions on its basis. This approach is extremely important for improving control and authorization-based mechanisms, justifying necessary resource support and developing comprehensive programs of development of isolated institutions, services and industry as whole.The term “conformity assessment” was introduced for the first time in the Federal Law № 184 of 27.12.2002 “On Technical Regulation”. The given law defines concept of conformity assessment as direct or indirect compliance with requirements imposed on the object. The concept presented in this law regulates industrial activity to objects and subjects of technical regulation. It is this ideology that became a basis of actual study, in which methodology and organizational technology was developed and experimentally tested to assess compliance of medical organizations and individual health services with requirements established by current legislation.Within study framework, development and experimental testing of system of assessing compliance of medical activities with requirements of regulatory documents was carried out. The object of the study was medical organizations and their structural divisions.The significant increase was established in specially developed integral compliance coefficients for studied objects for 2016-2020: from 76.5% to 82.0% for a number of medical profiles selected for the study and from 72.0% to 78.0% for medical organizations. The data obtained indicate that on the basis of a set of balanced indices, administration of monitoring objects made adequate management decisions.

The dynamics of indices of professional self-evaluation and social status of physicians: the first results of reforms in health care system

Abstract
In Russia, an active transformation of national health care system takes place and one of the most important of its activities became approval of the “road map”. In 2017, survey of physicians was carried out that revealed a number of patterns and problems in the field of physicians perception of their own lives, social self-implementation. In 2019, survey was re-carried out covered 311 specialists from the Republic of Bashkortostan. The purpose of this study was to analyze the dynamics of indices of professional self-assessment and social status of physicians in 2017-2019. The results testify a number of significant changes that took place by 2019: 1) number of specialists evaluating their own competence as “Excellent” statistically significantly increased (p=0.015); 2) percentage of specialists of second qualification category significantly decreased (p=0.0068) and previously observed relationship between professional self-assessment and qualification category lost its significance; 3) physicians began to statistically significantly more often identify themselves with highest stratum of society (p=0.012); 4) situation associated with lack of free time (p=0.021) and fears of losing job (p=0.044) worsened significantly. The research data mainly testify positive changes in social well-being of physicians in 2017-2019 and can be considered as first results of reform of national health care system. Nevertheless, increase of deficiency of free time and fears of losing job requires development and implementation of appropriate preventive measures.

The experience of reforming primary health care in conditions of demographic trends of average population age increasing

Abstract
The article presents experience of reforming of primary health care in the Belgorod Oblast in 2016-2019. In the course of reforming the original organizational model of interaction between doctors and patients developed and implemented. The measures were applied to strengthen the material-technical base and personnel potential of primary medical care. The offices of General Practitioners were arranged within step accessibility for patients. The continuous training of family physicians was organized at the workplace. The unified regional medical information system was developed and implemented enabling corporate conference communication, remote consultations, population feedback and remote arrangement of making appointment with physician. The differentiated approach to medical personnel remuneration of labor was brought on the basis of key performance indices that permitted to increase their salary and quality of work. As a result of the reforms, the percentage of people covered by family physicians dispensary observation increased by 13%, number of emergency calls decreased by 3.3% and number of calls because of illness reduced by 17%. The mortality rate of people older than able-bodied age began to decrease and population life expectancy reached prognosticated numbers of 74 years in 2019.

The social psychological aspects of professional education in health care: specifics of metacognitive processes

Abstract
The actuality of the study is linked with the fact that issues of social psychological aspects of professionalization of physicians and specifics of metacognitive processes that are the basis of professionalization and continuous university and post-graduate education are explored insufficiently. It is revealed that metacognitive processes in professionalization of physicians at different stages of education and profession mastering have a number of common and specific features. It is established for the first time that in medical students their metacognitive activity and independence in solving various communicative problems is related to increasing of socio-reflexion. At that, higher level of self-reflexion complicates realization of original solutions and excessive analysis brings doubts in success of creative solutions and results in overestimation of failure risks in the process of creative solution implementation. The analysis of post-graduate education stage proved that pediatricians focusing on cooperation with patients have higher level of self-control and self-organization. It is established that self-organization directly depends on motivation to obtain new information.

Abstract
The article presents the results of analysis of educational activities of a medical University. Among the issues that need to be resolved, quality of professional selection of applicants to medical universities, backwardness in time management skills, stress coping and leadership qualities in students are noted. The mechanisms for solving these issues are proposed on the basis of “puzzle technology” - embedding in existing matrix those activities that are capable to qualitatively change the situation in a particular field of higher education. Based on functioning of cluster technologies in the SEMC “Northern Caucasian”, joint measures are developed to optimize educational activities not only in educational organizations subordinated to the Ministry of Health of the Russian Federation, but also those in the structure of the Ministry of education and science of the Russian Federation that include medical faculties and/or institutes. The conclusion is made that the inclusion of educational organizations of any departmental affiliation into scientific educational medical clusters has positive effect on quality education of future physicians.

The article demonstrates, using a concrete example, how important the testimony of a contemporary is as a source of historical research. The testimony of this kind, supplementing archival materials, is capable to refute commonly accepted concepts and to bring the researcher nearer to understanding the real development of events. Thus, the letter of our former compatriot, whose life was saved in January 1942 by virtue of penicillin, urged author on further scientific research, that resulted in reconsideration of conception of time and circumstances of organization of first deliveries of antibiotic to the USSR. Also. the explanation was given to certain facts related to the history of its development and organization of production in our country.

A number of speeches of military physicians at the meetings of the military section of the congresses in 1914-1918 are analyzed. The ideas sounded at the Pirogov congresses were considered in the process of developing the Soviet health care system. The study of the materials of the congresses gives a notion of burning questions of military medicine of the studied period.

It is demonstrated that central position was assigned to analysis of integrated arrays of health statistical data of total population and its large groups. The individual health of personality as research object was shifted to periphery of interests of social hygienic science. On the example of works of S. A. Tomilin, the potential areas of enhancement of social hygienic comprehension of human health problematic are established.
