PRIORITY AREAS FOR THE DEVELOPMENT OF HEALTHY CITIES THAT CONTRIBUTE TO THE PRESERVATION OF THE HEALTH OF THEIR INHABITANTS

Abstract
Modern cities all over the world have various conditions and opportunities that ensure the preservation, strengthening and improvement of the health of their inhabitants. This is especially true in relation to the so-called healthy cities. Currently, the term «healthy city» means a city dominated by a rational physical, ecological and socio-economic environment that provides the vast majority of its inhabitants with opportunities for successful effective activities and a high-quality healthy life. The article presents international documents in the field of healthy cities development (World Health Organization, European Region). The achievements in the field of improvement of healthy cities abroad, contributing to the preservation, strengthening and improvement of the health of their residents, are described.

MALIGNANT NEOPLASMS OF THE REPRODUCTIVE SYSTEM IN MEN AND THEIR CONNECTION WITH THE HUMAN PAPILLOMAVIRUS. (REVIEW)

Abstract
The issue of malignant tumor occurrence associated with viral infections is a particularly significant one. Approximately 15—20% of tumors have some connection with various viral infections. An extensive amount of data has been gathered on neoplasms of the female reproductive system linked to the human papillomavirus (HPV). However, the connection between papillomavirus infection and malignant neoplasms in the male reproductive system requires further investigation. The aim of this review is to examine the association between malignant neoplasms of the male reproductive system and HPV, as well as to consider the issue of male vaccination. A research team conducted a literature review on the topic using databases such as the National Library of Medicine, Web of Science, and CyberLeninka. Data was collected on three types of malignant tumors in the male reproductive system: penile cancer, testicular cancer, and prostate cancer. In the context of the relationship between HPV and penile and prostate cancers, data on the presence of viral DNA in tumor tissue samples have been isolated. A study found that HPV DNA was present in semen samples from 34.9% of patients with testicular cancer, compared to only 2.4% in a control group. However, understanding of the HPV lifecycle raises questions about the link between HPV and testicular cancer. HPV multiplies in dividing epithelial cells, but testicular tissue is of mesodermal origin, and may not provide a suitable environment for HPV reproduction. Based on these findings, it is possible to draw the following conclusions: there are pathogenic reasons for the association between HPV and male reproductive system neoplasms. Further systematic review and meta-analysis are needed to fully understand this complex relationship.

TOBACCO CONSUMPTION DYNAMICS BEFORE AND AFTER THE COVID-19 PANDEMIC

Abstract

Purpose — to study Global Adult Tobacco Survey (GATS) indicators on tobacco consumption and the impact of the COVID-19 pandemic on them. Materials and methods. In 2022, 2,453 residents were surveyed (men — 1137, women — 1316) aged from 15 to 86 years (average age — 43,0 ± 16,4 years). There were 58,9% city dwellers, 41,1% villagers. The GATS technique was used. The results of the 2022 survey were compared with 2019. Mathematical and statistical processing included descriptive statistics and calculation of the χ2 distribution. Results. Over the past three years, the share of smokers has not changed (19,0% in 2022 vs 19,9% in 2019). At the same time, there were significantly more women smoking (7,4% vs 5,4%), and fewer men smoking (32,4% vs 36,7%). The number of smokers has significantly increased in the age group 65+, among women, as well as among persons aged 45—64 years. In 2022, smokers made fewer attempts to quit smoking. The number of users of electronic nicotine delivery systems increased (7.1% vs 5.4%), especially among men (9.5% vs 6.0%); a noticeable increase was observed in the 45—64 age group (2.8% vs 0.9%). Conclusion. During the COVID-19 pandemic, the proportion of smokers of both sexes in the older age group has increased significantly. The number of users of electronic nicotine delivery systems has increased. Many indicators of assistance to the population in quitting tobacco use and prevention of smoking among the adult population have worsened.

EFFECTIVENESS OF COLORECTAL CANCER SCREENING STRATEGIES: A REVIEW OF CURRENT SCIENTIFIC EVIDENCE

Abstract

Colorectal cancer is one of the most common malignancies in the world. Its incidence and mortality could be reduce through primary prevention activities, which include healthy lifestyle behavior and avoidance of risk factors, and through screening (secondary prevention). Understanding the strengths and weaknesses of different colorectal cancer screening strategies is within the scope of interest of different health professionals. Therefore, the aim of this review is to synthesis the current scientific evidence that addresses the issues of effectiveness of population-based colorectal cancer screening programs and new methods for its early diagnosis.

Abstract
The personnel shortage in domestic medical organizations continues to be a key problem in healthcare. Modern research in the field of personnel policy reflects a predominantly random choice of specialty by a graduate after graduating from a medical university — 30% of young specialists leave the profession, often noting that the specialty «did not suit». There is a growing need for job profiling in order to more effectively distribute personnel in various sectors of economic activity.

SOURCES AND CONSEQUENCES OF HIGH SALT CONSUMPTION IN MOSCOW: A STUDY OF DIETARY HABITS

Abstract
The aim of this study was to examine the level of salt consumption among various population groups in Moscow. The study was conducted using the 24-hour dietary recall method (24hDR) and the food frequency questionnaire (FFQ) over the course of one year. The results showed that 49.6% of Muscovites have excessive salt consumption, with this phenomenon being more prevalent among men (60.3%) compared to women (41.9%). Data analysis revealed significant consumption of high-salt foods and dishes, highlighting the need for monitoring and adjusting salt intake in the diet to prevent hypertension and cardiovascular diseases.

RESOURCE PROVISION OF PSYCHIATRIC AND PSYCHOLOGICAL CARE IN MOSCOW IN 2009—2023

Abstract
The authors analyze the number, dynamics and structure of medical personnel providing psychiatric and psychological care in organizations subordinate to the Moscow Healthcare Department in 2009-2023, evaluate the changes in the bed fund and the number of visits, and predicts the number of medical personnel at the end of 2025. It was concluded that the resource provision psychiatric and psychological care in Moscow is at a high level, and in 15 years the number of employed doctors in the field has increased by 26.44%. The changes in the structure were influenced by a decrease in the number and proportion of psychotherapists and an increase in the number of psychologists. Psychiatric and psychological care in the capital has changed its focus from inpatient therapy to inpatient replacement: the number of discharged patients from day hospitals in 2013-2023 increased in 2.42 times, and at the same time, over the same period, the hospital bed stock and bed days spent, on the contrary, decreased by 46.04% and 45.70%, respectively. It is expected that by the end of 2025, the number of medical personnel in the industry will increase by 12%.

HEALTH SCHOOLS — MOSCOW EXPERIENCE IN IMPLEMENTING A HEALTH EDUCATION PROJECT

Abstract
A unique “Health School” project has been implemented in Moscow to teach older people knowledge about health, prevention of chronic non-communicable diseases, injuries and poisonings. The project was based on the integrative interaction of the Department of Health and the Department of Labor and Social Protection of Moscow. The leading role in the implementation of the project was played by employees of the Research Institute of Healthcare Organization and Medical Management of the Moscow Department of Health. The “Health School” project was successfully implemented in 2023 and its implementation continues in 2024. Training older people in the basics of health conservation has proven its relevance and effectiveness. A promising area of development could be the “Evidence-Based Happy Longevity” Platform, a knowledge base on active longevity issues and “best practices” in this area.

STUDY OF PREVENTION STRATEGIES FOR SIX RISK FACTORS IN REDUCING MORTALITY FROM NON-COMMUNICABLE DISEASES 25 × 25

Abstract
Non-communicable diseases (NCDs) continue to progress against the background of an increase in risk factors and the emergence of new ones, such as postcovid syndrome. Due in part to this influence, the progression of the atherosclerotic process and deterministic cardiovascular diseases is observed. In this regard, the aggravation of NCDs remains alarming against the background of the influence of six leading risk factors «25 × 25». The article presents a review of the literature and the conceptual directions of these leading factors as dominant in the development of NCDs. The directions of secondary prevention are considered and the expediency of their early implementation is justified. The presented methodological bibliometric method made it possible to analyze the experience of a number of countries that led to a decrease in the development of NCDs. The methods of bibliographic data search included search queries on the Scopus Web of Sciense, MedLine, The Cochrane Lybrary, PubMed databases. It is noted that achieving even six goals with the correction of risk factors will reduce morbidity and mortality from such four major NCDs as cardiovascular diseases, chronic obstructive pulmonary disease, stomach cancer, diabetes mellitus by 2025 to levels close to the target of «25 × 25», reducing by 77% the gap between the growth situation or stagnation and trends in the growth of new risk factors.

Abstract
The specificity of the medical profession, which involves constant communication with sufferers and their loved ones; the need to quickly master rapidly developing medical technologies and the digital environment; organizational and economic changes in the healthcare system that are increasingly intensifying the work of medical workers create a risk of expanding the scope and aggravation of the problem of professional burnout. The article presents an analysis of approaches to identifying signs and classifying the stages of professional burnout among medical workers, as well as methods for preventing and relieving this syndrome in domestic and foreign practice. In addition, the results of two mass surveys of medical specialists employed in the Moscow healthcare system, conducted at five-year intervals, are presented.

Abstract
The purpose of the study is to identify the features of the emotional component of professional deformation (burnout) of healthcare workers in the post-covid period. A survey was conducted of 658 healthcare workers with characteristics of the emotional component of professional deformation (burnout) in the post-covid period. Questionnaires of emotional burnout and self-assessment of emotional states were used. Mathematical and statistical processing included the K. Pearson correlation criterion and the Chaddock–Snedecor scale.

ISSUES OF ROUTING PATIENTS WITH AUTOIMMUNE HEPATITIS

Abstract
Autoimmune hepatitis is a chronic, immune-mediated liver disease characterized by an increased level of immunoglobulins G and/or gamma globulins, the presence of autoantibodies and a typical histological picture. Diagnosis of autoimmune hepatitis causes difficulties due to various variants of the course, wide variability of the clinical picture, which leads to a large number of missed cases of the disease. Treatment of аutoimmune hepatitis is aimed at reducing the activity of an inadequate response of the immune system against cells of its own body and achieving complete remission of the disease. The timeliness of starting treatment is directly related to the prognosis of the disease. These features of the disease necessitate proper patient routing and shortening their path to specialists with knowledge and experience in managing this category of patients.

Abstract
Knowledge of the epidemiology of Duchenne muscular dystrophy is advisable for planning measures to improve the diagnosis and treatment of this disease. Purpose: to study the epidemic situation of Duchenne muscular dystrophy, as well as the average age of its diagnosis in the Russian Federation. The problem of accuracy of statistical estimates is due to heterogeneous diagnostic criteria. The development of unified diagnostic criteria (male gender and mutation carrier) is required. It is advisable to consider the issue of screening the level of creatine phosphokinase in young children with subsequent targeted examination of children with high levels, including testing for carriage of the Duchenne muscular dystrophy mutation.

CURRENT ISSUES OF IMPORT OF MEDICAL DEVICES AND EQUIPMENT TO THE RUSSIAN FEDERATION

Abstract

The import of medical products remains an important topic at present, given the relevance of healthcare in connection with the sanctions policy of Western countries and the ambiguous situation with foreign supplies of medical products. The situation with the dominant share of imported medical products in the Russian market highlights the dependence on international supplies and technologies in the healthcare sector. The policies and sanctions imposed by the European Union and other countries regarding the supply of medical equipment to Russia cause certain restrictions and difficulties due to the current political situation. Although these goods are classified as vital and do not fall under sanctions, many companies from Europe and the United States have suspended deliveries due to the so-called «moral embargo». Lack of access to modern equipment and medicines can lead to a deterioration in the quality of medical care, an increase in waiting times for treatment, as well as possible complications and a deterioration in the prognosis of the disease in patients. The article analyzes the dynamics of the medical products market over the past few years, and identifies changes in the structure of the medical devices market. The share in the import of medical equipment to Russia was also studied. In addition, the prospects for the development of the medical industry are considered and the current situation is assessed. To solve the problem in the short term, the necessary measures have been proposed to include vital medical devices and equipment in the list of the Ministry of Industry and Trade for its import according to the rules of parallel import.

ANALYSIS OF INFORMATION DEMAND FOR MEDICAL SERVICES ABROAD IN THE RUSSIAN FEDERATION (2018—2023)

Abstract
The authors analyzed information of the demand for treatment abroad in search engines Yandex and Google the Russian audience of Internet users over the period from 2018 to 2023, covering the 3 stages of the development of international medical tourism market: before pandemia (2018—2019), pandemia (2020—2021) and sanctions (2022—2023). Based on dynamic indicators, a forecast is given for strengthening the positions of individual countries in the information Internet space of the Russian medical tourism market, and the main directions of integration processes in the medical tourism market in the space of the member states of the Commonwealth of Independent States are outlined.

CORRELATION OF COGNITIVE IMPAIRMENT WITH PHYSICAL AND PSYCHOLOGICAL HEALTH IN VARIOUS DEMOGRAPHIC GROUPS

Abstract
Within the framework of this study, the correlation of cognitive impairment with physical and psychological health in various demographic groups was studied. The analysis is based on data collected through a survey of 320 participants, which allowed us to assess the impact of age and gender differences on cognitive functions. A comprehensive approach was applied, including a decision tree and a random forest to identify key predictors of disorders, as well as correlation and regression analysis methods to determine statistically significant links between cognitive functions and other variables such as physical activity level and psychological state. It has been found that physical limitations and psychological conditions such as anxiety and depression have a significant impact on cognitive abilities.

Abstract
The authors digitized therapeutic factors used in the treatment of various forms of cognitive disorders, including dementia, with the subsequent development of a medical information system «Diagnosis and treatment of cognitive disorders». The medical information system «Diagnosis and treatment of cognitive disorders» was created on the basis of the 1C platform, supplemented by the module «Treatment of cognitive disorders», focused on supporting medical decision-making when choosing therapeutic factors for patients with cognitive disorders. It is planned to use this module in the complex activities of memory impairment classrooms.

HEALTH AND SATISFACTION WITH THE HEALTH CARE SYSTEM AS FACTORS IN THE QUALITY OF LIFE OF THE OLDER GENERATION OF METROPOLITAN RESIDENTS

Abstract
In the context of an aging population in megacities, it is important to maintain the health and activity of older people, improving their quality of life. To do this, it is necessary to study the factors affecting longevity, including individual health characteristics and perception of the health system (accessibility, responsiveness, effectiveness, ageism). The purpose of the study: to identify an assessment of the quality of life of elderly Muscovites and the influence of factors related to health and the assessment of the health care system on it. Materials and methods: a sociological survey, descriptive statistics, correlation analysis, and a z-test were used to compare the proportions of variables. Results: an assessment of the quality of life and its components based on questionnaires showed that health is a key factor for the elderly, although its assessment was low. 47% of the elderly are committed to healthy lifestyle, of which 40% regularly exercise. 25% have bad habits, 26% are disabled, which reduces the quality of life. 18% seek medical help on a monthly basis. Those who go to doctors less often rate the quality of life higher (p m 0.001). Satisfaction with the last medical visit is higher than with the healthcare system as a whole. Conclusion: the study revealed a link between healthy lifestyle and the quality of life of the elderly. Regular physical activity improves quality of life by reducing hypochondria and potentially offloading the healthcare system. Satisfaction with the health care system also affects the quality of life and requires an increase in the availability of medical care.

AGEISM IN THE DOCTOR—PATIENT RELATIONSHIP

Abstract

The presented work examines the problem of ageism in the doctor-patient relationship, as well as the impact of this discrimination on the quality of medical care and the psychological state of patients. Ageism is defined as bias or discrimination based on age, which can manifest itself in various forms, including underestimation of symptoms in older patients and negative stereotypes towards younger patients. The text emphasizes that ageism violates trust between a doctor and a patient, which leads to a deterioration in communication, a decrease in the quality of diagnosis and treatment, as well as to the rejection of medical recommendations by patients. Based on the analysis of international practice, the main strategies for combating ageism were identified, such as the introduction of educational programs for medical professionals, the development of a state anti-ageist policy, the creation of an inclusive environment in medical institutions and public campaigns.

SPECIFIC FOREIGN TOOLS FOR ASSESSING THE QUALITY OF LIFE OF PATIENTS WITH COPD

Abstract
Introduction. Patients with chronic obstructive pulmonary disease (COPD) usually have a decreased quality of life (QOL). Specialized questionnaires on QOL may be more sensitive when studying individual issues of COPD therapy than general (universal) questionnaires. Goal. Conducting a review of specific tools for assessing the quality of life of patients with COPD. Materials and methods. The study was carried out based on the results of a search of scientific literature presented in the bibliographic databases PubMed (USA) and ELIBRARY.RU (Russia, Russian-language sources). Keywords were used to search for relevant sources: «quality of life», «COPD», «chronic obstructive pulmonary disease» etc. In order to search for information on individual questionnaires, queries containing the names of the questionnaires were used. Additional references were obtained using the search engines Google and Yandex. Results. Based on the results of a study of the scientific literature and a search in the PubMed / Medline information base, 22 specific tests used to assess the quality of life of patients with COPD were considered. These tests demonstrate different relevance and scientific applicability in recent years. Five tests were most often used in foreign and domestic studies: St. George respiratory questionnaire (SGRQ), the COPD assessment test (CAT), the modified Medical Research Council dyspnea scale (mMRC), the chronic respiratory questionnaire (CRQ), the clinical COPD questionnaire (CCQ). Conclusion. Further study as well as local adaptation (if so required) of some questionnaires will open up new opportunities for their wider implementation into the routine daily practice of providing medical care to patients with COPD in the regions.

All over the world, with regard to digitalization in the provision of primary health care, issues of the principles of the cost-effective resource system for the provision of medical services covering all residents (any category of the population), ways to achieve other goals set for the health system at the territorial or regional level are being discussed. Depending on the main goals and principles set out in national legislation, including legislation in the field of health protection, approaches to achieving the Sustainable Development Goals and, accordingly, approaches to organizing the digitalization of medical care differ in different States. The issues of automating the processes of a medical organization and generating relevant information relate to the ways of using artificial intelligence to free up time for doctors, reduce the administrative burden on medical service providers. The level of digital medical literacy, combined with modern possibilities of remote interaction in the patient-medical worker format, can be decisive at the first stage of seeking medical help — with remote patient triage, which is an indispensable tool for determining both the need for assistance and the urgency of providing assistance. The experience of remote triage of patients by non-medical staff provided by the NHS Pathways clinical decision support system, used by the National Health Service of England, has freed up general practitioners (doctors and nurses) to provide other forms of medical care. In 2005, nurses and, to a lesser extent, general practitioners in England accounted for 100% of the staff handling telephone calls in the similar NHS Direct remote consultation system. The foreign experience of patient teletriage in our country will be limited by the legislation of the Russian Federation in the field of health protection, however, some existing restrictions, taking into account the assessment of risks to patients brought by innovative development, may be revised during the implementation of the «Program of experimental legal regime in the field of digital innovations in the field of medical activity, including the use of telemedicine technologies and technologies for collecting and processing information about the state of health and diagnoses of citizens», which will expire on 08/01/2026.

EXPORT OF MEDICAL EDUCATION IN THE WORLD MARKET OF EDUCATIONAL SERVICES

Abstract
The article examines current trends and challenges related to the export of medical education in the global educational services market. The analysis of the data on the number of foreign students at medical universities leading exporting countries, including USA, UK, Australia, Canada, Germany, Russia and China. The factors influencing the choice of a country for study, such as the quality of educational programs, tuition fees, and the availability of visa and immigration procedures, are highlighted. Special attention is paid to the impact of the COVID-19 pandemic and the growth of online education on international student mobility and accreditation of medical programs. In conclusion, recommendations are proposed for exporting countries to strengthen their positions in the global medical education market.

Tysberg, which is formulated as the ratio of the results that are important to patients to the costs incurred to achieve these results. Abstract
Many countries around the world are faced with the problem of a shortage of medical workers due to insufficient investment in medical education, inconsistencies between training and employment strategies for medical personnel and the needs of the healthcare system, complicated by the lack of human resources for healthcare in remote areas and the migration of medical workers. One of the tools for developing medical personnel and forming a stable workforce is mentoring, which should be used throughout the working life of employees, and not only during the adaptation process of young employees. The purpose of the study is to identify the role of mentoring in reducing staff turnover and forming a sustainable workforce.

Against the background of a large-scale modernization of primary health care, introduction of new treatment and diagnostic methods and clinical guidelines, primary care physicians have an increasing need to improve professional skills, expand horizons and adapt to a rapidly changing reality, requiring the development of meta-skills, for example, critical thinking and research competencies. The purpose of the study was to assess the possibility of developing meta-skills among primary care providers through an educational project exemplified by the “Scientific Laboratory: Moscow Polyclinic” urban project.
