Today, the total fertility rate in all European countries is below the minimum required to ensure population reproduction in long term - 2.1 children per woman during her life. According statistic data, 4.167 million children were born in the EU in 2019 that corresponds to 9.3 births per 1000 of population. In the EU, 1.53 children went to one woman in 2019 (in 2018 - 1.54). The lowest total fertility rates in 2019 were registered in Malta (1.14), Spain (1.23) and Italy (1.27). In the United States, fall of rate of childless women with higher education disputes well-established observation of positive relationship between education and childlessness. The infertility affects approximately 8-12% of couples worldwide, at that in developed countries, up to 15% of all couples are infertile. In connection with the problem of birth rate reduction in developed countries, researchers highlight such problems as increasing of average age of mothers at delivery (30-35 years and older) and intervals between labors, development of subsidiary reproductive technologies, pregnancy outcomes, newborns health in short and long perspective. The global COVID-19 pandemic resulted in resources deficiency, complexities with medical services support related to reproductive health, risks for maternal and perinatal outcomes in pregnant women with COVID-19. The female health: status, causes of deterioration, measures of improvement
Sadykov R.M., Bolshakova N.L.
Abstract
The deterioration of health status of female population is recognized as one of the important medical social problems. On its resolution depends reproduction of population and improvement of social demographic indices in the Russian Federation. The female health can be considered as one of the indices of physical, spiritual and social well-being of family. One of significant aspects of considering importance of female health status is her attitude to issues of her own health. The situation is aggravated by increase of asocial behavior in young women, accompanied by decreasing of quality of life and significant loss of moral values. Besides, organization of treatment and prevention measures for early detection and reduction of gynecological morbidity does not meet modern requirements. The economic conditions and ecological situation negatively contribute to deterioration of female health. Such social factors as lack of work or threat of losing it, low per capita income, inadequate housing and poor-quality nutrition, harmful habits deteriorate health status of female population. The status of one's own health and disease prevention are in last place in female population. In modern conditions, due to employment overload, women pay no due attention to their own health, do not undergo medical examinations, rarely visit physicians as study results confirm. The female health protection is facilitated not only by medical and diagnostic measures, but also by wide range of preventive activities, formation of healthy lifestyle, favorable conditions for education, work and life, legal and social support of women during child-bearing and child-rearing. The female health protection goes far beyond scope of medical problem and requires corresponding management decision-making and inter-departmental measures on legal and financial support of motherhood and childhood. Therefore, protection and promotion of female health, including reproductive health, are necessary conditions of achieving national security and improving demographic indices of the Russian Federation.

The life-span of population of the Irkutsk Oblast
Leshchenko Y.A., Lisovtsov A.A.
Abstract
The indicator of average life expectancy at birth is widely used as systemic indicator of population quality of life and health. The purpose of the study is to identify, to evaluate quantitative and structural changes of indicator of life expectancy at birth in population of the Irkutsk Oblast during 1990-2019.The data of life expectancy at birth was obtained from the Rosstat websites. The original calculations of survival tables were carried out using the databases of the Center for Demographic Research at the New Economic School. The analysis of trends was carried out using linear regression analysis. The analysis of role of exogenous and endogenous determination of mortality in formation of average life expectancy was carried out.In 1990-2005, in the Irkutsk Oblast, occurred change in ranking of causes of death that determined the greatest irrecoverable losses of population and reduction of indicator of average life expectancy at birth. The essence of this change is in abnormally high proportion of exogenous causes of mortality, especially class of external causes of mortality, in the structure of irreversible demographic losses. In the Irkutsk Oblast, in 2006-2019, occurred favorable changes in the structure of irrecoverable losses, testifying returning to modern type of mortality. During the same period, indicator of average life expectancy at birth increased. It is established that changes occurred since 2006 in the quantitative and structural indices of mortality and average life expectancy at birth are very positive. However, during demographic processes in 2020, the COVID-19 pandemic intervened. This factor is bringing negative impact on economy, and after that on social sphere and public health. The further course of development of social demographic situation in the Irkutsk Oblast, in particular, dynamics of indicator of average life expectancy at birth, will depend on success of activities of authorities of all levels in struggle with pandemic and overcoming crisis events in social economic development.

Kalininskaya A.A., Lazarev A.V., Allenov A.M., Merekina M.D., Kizeev M.V.
Abstract
The primary health care is characterized by its directivity to population health, while general practitioner is key figure in its organization and is responsible for assigned population health. The Russian Federation continues to suffer from overspecialization of medical care in outpatient health care. At that, the number of unreasonable visits to medical specialists is too high. The general practitioner, with appropriate training, can appoint patients by 11 specialties and in conditions of municipal polyclinic can undertake up to 81% of visits to medical specialists. The purpose of the study is to evaluate primary health care functioning in Moscow and to formulate proposals for reforming health care, considering development of general medical practices in a metropolis. The study was focused on analysis of morbidity of population of Moscow, on support of out-patient health care with physicians before and after implementation of three-level system of ambulatory polyclinic care, on evaluation of preventive activities of general practitioners and on development of proposals for improving organizational forms of activities with emphasis on health preservation. In the study were applied such methods as statistical, direct observation, sociological. The analysis was applied to forms of federal statistical observation i.e. form № 30 in Moscow and the Russian Federation for 2010-2019. The population sociological surveys concerning satisfaction with medical care were carried out. The research base was Moscow. The analysis demonstrated that results of Moscow health care system reformation is economically effective because of shortage of medical personnel primarily of administrative staff and its rational employment. The reformation results were positively assessed by patients, whose satisfaction with medical care made up to 95.6%.The general practitioner is to be responsible for health status of attached population and is called upon not only to carry out treatment and preventive activities, but also to coordinate organization of medical care of attached population at all stages of its provision. The implementation of general medical practice in conditions of municipality is to calculate complete transition of medical organizations providing out-patient medical care to general medical practice, that will result to certain extent, in reduction of staff positions of medical specialists. It is necessary to expand duties of medical nurse in general practice. The optimal model of joint work of general practitioner with feldsher and medical nurse of general practice. The complex general practice team can include medical registrar. The proposed perspective models permit to implement integrating role of general practitioner and to increase accessibility and efficiency of primary health care.

Grynin V.M., Avetisyan A.Y., Reshetnikov V.A.
Abstract
To improve quality and efficiency of primary health care and to strengthen its preventive directivity is possible through health care modernization including implementation of management technologies, development of database for evaluation and control. The purpose of the study is to develop method of evaluating activity of stomatologic clinic. The methodology was based on concepts of “functional systems” and “decision making”. The listing of informative criteria was based on data analysis of functioning of stomatologic service of the Chechen Republic in 2002-2016 and results of sociological survey of stomatologists (n=181), health care administrators in stomatology (n=45), paramedics (n=220) and patients (n=359). The experts (n=13) developed required number of criteria and their gradation. The algorithm of evaluating activity of stomatologic clinic was developed using systematic approach. The listing of 52 criteria and their 10 blocks were established. The experts established coefficients of significance of criteria (0.0084-0.0781; ≥0.0224 - more significant, <0.0224 - less significant). Three levels were determined for each criterion (according principle of minimization) that became a basis for calculation of integrated indicator of efficiency of functioning of stomatologic polyclinic. The expanded methodological approaches ensure comparartivity of estimates of stomatologic polyclinic in various time periods. The health care authorities have opportunity to objectify analysis of functioning of a number of polyclinics in different periods.

Kaliev M.T., Imankulova A.S., Djumalieva G.A., Azimzhanova M.N., Tolbashieva G.U., Naraliev U.T.
Abstract
The health care reforms in the Kyrgyz Republic are directed to develop hospital financing system. In 2003 the system of clinical-cost groups was introduced. The surgical clinical-cost groups are organized on the basis of existing classifier “The Surgical Operations and Manipulations”, which is not revised since its approval in 1997, that resulted in distortion of statistical information. Hence, need in development and implementation of modern reference book is obvious. Zhuravskii S.G., Goncharova S.G., Poddubny M.V.
Abstract
The letters of S. P. Botkin from Bulgaria (1877) to his wife, E. A. Botkina (née Princess Obolenskaya), were published as travel diary in the journal “Vestnik Evropy” in 1892 and as separate book in 1893. The family events related to publication of epistolary were restored. For publication, material of private nature was abridged by E. A. Botkina. Then, passages with compromising criticism were removed by editor of the journal. In spite of good quality of prepared text, publication was postponed by E. A. Botkina herself and afterwards it was lingered on because of claims of censorial department. The role of N. A. Belogolovy, public figure and friend of S. P. Botkin of many years and M. M. Stasyulevich, a publisher, in final decision to publish epistolary is emphasized. The comparison of original and printed matter carried out for the first time, established that cuts made up to more than one third of author's text.
