ON THE NEED TO TRAIN MEDICAL SPECIALISTS TO WORK IN THE CONDITIONS OF THE INTRODUCTION OF REMOTE TECHNOLOGIES

Abstract
The development of telemedicine and the provision of medical care using remote technologies requires appropriate competencies and skills from medical specialists, the formation of which is an important task of adapting educational systems to modern challenges and responding to requests from both medical staff and patients. Based on the analysis of foreign scientific publications, the paper considers the assessment of the need to train medical workers in remote work with patients and the prospects for the introduction of telemedicine consultations into everyday professional practice. It is concluded that having more knowledge about which aspects of medicine are best combined with telemedicine, it is important to train future medical professionals in the use of these technologies and the provision of these types of medical care. Doctors should not only be trained in the use of telemedicine, but also learn how to do it professionally, safely and on the basis of evidence.

PASSPORTING OF PARENTS OF DISABLED CHILDREN AS AN ANALYTICAL RESOURCE OF SOCIAL POLICY

Abstract
The article presents the results of a study of the main social and household characteristics of families with disabled children, conducted in the territory of the Belgorod region (targeted survey; n = 990). The research made it possible to answer questions about children's health problems, their living conditions, the economic status of the family, the work of parents, the social environment and other characteristics of families. The article examines the passporting method of target problem (socially vulnerable) groups as an effective way of obtaining system information, a social picture of the situation, conditions, and problems of such families. The results of the research made it possible to determine the risks associated with the condition of children who require constant help and support, the category of families with unsatisfactory living conditions, the need for advanced training and retraining among parents. An important result of the study was the income map of families with disabled children, which identified the problematic characteristics of the socially vulnerable group and helped determine the directions of primary assistance and support. The scientific result of the article is the conclusion about the need for systematic control of all problematic components of the life of families raising disabled children, about the need for monitoring such information for the organization of targeted assistance and changes in social policy to meet the actual needs of such families.

Abstract
The article discusses the psychological and pedagogical foundations of the general educational process in a medical educational institution and the formation of professional training of a modern doctor who is able not only to conduct medical and diagnostic activities, but also to provide psychological and pedagogical assistance. The reorientation of the doctor's activity taking place in modern society from the position of treating diseases to the self-preserving behavior of patients, which aims to preserve human health by preventing the development of diseases and pathologies, involves the involvement of medical specialists in the educational process. The relevance of the topic of the article is due to the need for the formation of psychological and pedagogical competencies of medical specialists receiving higher medical education, despite the complexity and diversity of the doctor's activities. Despite the fact that a sufficient number of legislative and social programs have been implemented by state structures in recent years, which aim to popularize a healthy lifestyle among the residents of our country and reject negative factors, the main obstacle to educating citizens to health-saving behavior is the weak motivation of doctors to use psychological and pedagogical competencies in professional activities. It is obvious that medical diagnostic activity at the present stage of development requires special training from graduates of medical educational institutions, including the basics of a psychological and pedagogical nature. The purpose of the study is to consider the psychological and pedagogical readiness of a doctor after graduation from a medical school for professional activity, to assess professional and business communications in a doctor-patient relationship and the ability to perceive differences between patients of a social, ethnic and cultural nature.

SOCIAL REPRESENTATION OF PERSONS WITH DISABILITIES IN MARKETING AND ADVERTISING PRACTICE

Abstract
The article discusses the issues of including the image of persons with disabilities in marketing and advertising practice. The data reflecting the statistics of disability and consumer behavior in the market of goods and services for the disabled and persons with disabilities are presented. The purpose of the study is to analyze the current practice of marketing and advertising activities using the image of disability and persons with disabilities. It is concluded that both advertising and any other forms of marketing initiatives reflect society, and therefore most advertisers at some point create communication products that somehow reflect their target audience. Good examples of images of people with disabilities and a wellthought-out advertising campaign are designed to increase brand loyalty and at the same time make the product more popular with a wide audience.

POLIZEISTAAT AND POLICE AGAINST PESTILENCE: RUSSIAN AUTHORITIES AND LAW IN THE FIGHT AGAINST THE THREAT OF EPIDEMICS IN THE 1st THIRD OF THE 18th CENTURY

Abstract
The beginning of the 18th century in the history of the Russian state and society was marked by the beginning of large- scale transformations initiated by Peter I. As a result of these reforms, the patriarchal Moscow kingdom turned into a “regular” Russian empire, the political, legal and administrative structure of which differed significantly from what it was before. It is no coincidence that Peter is often referred to as a «revolutionary on the throne.» In his actions, Peter was guided by the idea of the “common good” and the ideas of the “regular” state and Polizeistaat. He drew them in Western Europe, in the countries of the Protestant circle — Prussia, Denmark, Sweden. One of the leading ideas of the Polizeistaat doctrine was the state's active policy to form a welfare society, which was supposed to be achieved through the active intervention of the authorities in the daily life of citizens, its regulation and regulation through various regulations, instructions and instructions. A regular, properly organized police force was one of the most important instruments of such intervention, which had as its ultimate goal the creation of conditions for the growth of the well-being of subjects and their number. This growth in the number of subject population was also achieved through the creation of a system of measures to prevent epidemics and their consequences. MATRIMONIAL BEHAVIOR OF STUDENT YOUTH IN THE CONTEXT OF SOCIAL HEALTH STUDYING (GENDER ASPECT)

Abstract
The article presents the results of a research work aimed at identifying the features of the matrimonial behavior of students in the aspect of social health studying. Of particular importance is the study of students' value attitudes towards marriage and family in a gender context. The authors conducted an empirical study on the basis of the National Research University «BelSU» (n = 267; proportional selection based on gender, age, course, level and form of education). The analysis of the main marital attitudes of this group (attitude towards marriage, readiness for marriage, criteria for choosing a marriage partner, motives for marriage, reproductive behavior, reproductive health) was carried out on the basis of primary data obtained during a survey of students of the Belgorod State National Research University. It was revealed that most of the students of NRU «BelSU» have a traditional idea of marriage, have a desire to marry and start a family, students prefer official marriage, which implies traditional family values, which characterizes positive trends in the formation of social health. When choosing a marriage partner, most young people are guided by the personal characteristics of a person and their value orientations. It was revealed that the formation of the students' social health, among other aspects, is due to the orientation towards the established values in relation to the value of marriage and the socio-cultural norms of matrimonial behavior.

LABOR STRATEGIES OF GRADUATES OF MEDICAL EDUCATIONAL INSTITUTIONS: PREDICTED RISKS OF THE MEDICAL INDUSTRY

Abstract
The article presents the results of a study of the labor strategies of youth included in the personnel reproduction of regional health care. The authors of the article consider the results of a complex sociological study of graduates of educational medical organizations and specialized medical classes (focus group studies, target survey; n = 1027). The research made it possible to answer questions about the dynamics of professional motivation of young people, to determine labor strategies and reasons for choosing in life planning. The article proposes a factor model of decision-making when choosing a workplace by graduates of medical specialties, gives an assessment of existing workplaces in the regional health care system, conducts an audit of the need for stimulating measures to retain and secure young personnel in the medical industry. The results of the research made it possible to determine the risks of youth labor strategies as a personnel reserve of medicine and to propose directions for their leveling and correction of the situation. An important result of scientific research can be considered the proposed diagnostic complex itself, which makes it possible to establish the dynamics of indicators during subsequent measurements (motivational transformation, change in migration aspirations and labor choice) and to determine the effectiveness of the measures and decisions taken. The result and scientific result of the article is the conclusion about the lack of orientation to the key problems of the implementation of labor strategies by graduates of educational medical organizations from the branch administration system, early pre-professional burnout of young personnel, the problem of regional public opinion regarding the prestige of the medical profession and effective labor strategies of future doctors.

Abstract
The article considers theoretical approaches to determining the essence and characteristics of the organization of entrepreneurial activity in the field of public health. The emphasis is on innovative entrepreneurship, which involves entering new, often unknown or untested markets, which are characterized by high uncertainty. Innovation-driven entrepreneurship does not necessarily require new high-tech advances — rather, it is a new recombination that creates a new way of doing a certain set of activities. It is noted that intrapreneurship, which is based on innovative developments of employees of medical organizations, is also a form of entrepreneurial activity in the field of public health.

THE ROLE OF EMPLOYERS IN ENSURING WORKING CONDITIONS CONDUCIVE TO THE HEALTH OF MEDICAL WORKERS

Abstract
The article examines the normative, conceptual and ethical aspects of employers' responsibility for creating jobs that reduce risks and ensure the health of medical workers. An overview of the normative documentation of the World Health Organization and the International Labor Organization is presented, data reflecting the morbidity of health workers due to a weak degree of protection in the workplace are presented. It is concluded that the high degree of not formal, but real responsibility of the employer for ensuring working conditions conducive to the health of medical workers has a positive effect on their loyalty, reduces staff turnover, ensures the effectiveness and sustainability of the organization in the medical services market.

Abstract
Increasing interest in society in topics related to medicine and health is due to the current trend to consider physical, mental and social well-being as a basic human need. In the context of modern life, health is perceived as one of the most important values of an individual and society as a whole, therefore, medical discourse today plays a special role in culture, covering not only the professional expert and scientific community, but also the widest audience — the audience of the media. The media play a key role as a source of medical information and its targeted and targeted distribution. In this article, the medical discourse is analyzed in terms of its formation as a social activity and the role of modern mass media in it. The aim of the study is to analyze the demand for medical discourse in society and the features of its mediatization, which is the most important factor in the value-semantic dynamics of society and its way of life. The main empirical material is publications in the magazine “Zdorovie”, the most popular Russian publication about a healthy lifestyle, medicine, practices for the treatment and prevention of diseases, the art of preserving beauty and vitality. PROTECTING THE LIFE AND HEALTH OF CUSTOMS OFFICIALS FROM THE HARMFUL IMPACT OF RADIATION MATERIALS

Abstract
Radiation materials pose a serious danger to human life and health, especially for personnel who interact with such materials in the performance of their duties. The effect on the human body of ionizing radiation that emits radiation material manifests itself in the form of «radiation sickness», when all human organs are affected, and radioactive rays can cause gene mutations that can be inherited in the form of deformities or serious diseases. For this reason, it is very important to ask whether the radiation safety of officials of customs authorities is ensured at a sufficient level, who, when carrying out customs control of such materials, endanger their lives and health. The analysis carried out in the study testifies to the disparity in the requirements and established rules for ensuring the safety and protection of the health of customs officials working with radioactive materials. In this regard, this article studied the risks of the negative impact of radiation materials on the health of a customs inspector during customs control, studied in detail the process of organizing the work of group «A» personnel who have access to work with such materials, and formulated appropriate conclusions and recommendations for eliminating imperfections in ensuring the radiation safety of customs officials from the harmful effects of radioactive materials in order to protect the life and health of the inspector from the radiation threat in the performance of his duties.

PROBLEMATIC ASPECTS OF CUSTOMS REGULATION OF IMPORTED COMPONENTS OF MEDICAL DEVICES

Abstract
In modern conditions of sanctions policy, the issue of import of various categories of goods is subject to special control and attention from the state. This aspect may be due, on the one hand, to the introduction of retaliatory measures related to the prohibitions and restrictions imposed on the import of certain goods, and, on the other hand, to the development of a special approach to customs clearance of products, manifested in the simplification of requirements for the provision of accompanying documents. Of course, such a category of goods as medical devices, being the most important from the point of view of its intended purpose, requires close attention from the state, especially due to the fact that a significant amount of medical devices and their corresponding accessories are imported from third countries. The article discusses in detail the basic requirements for documents that must be accompanied by imported medical devices, examines the key aspects of customs clearance of this category of goods, and highlights problematic issues related to the import of medical device accessories, on the basis of which proposals were formulated for their settlement.

Abstract
The article is devoted to the analysis of the mechanisms of state support for the rehabilitation of disabled people. Given the limitation of the functional state of people with disabilities, providing them with rehabilitation services is an important component of maintaining and preventing the deterioration of their physical condition and health. In addition, taking into account the limited opportunities for work, rehabilitation and social support measures are an important part of the material support of the disabled and an important measure to prevent poverty among them by the state. The article discusses the main components of the mechanism for the rehabilitation of people with disabilities in Russia, explores the impact of the measures and programs taken on various aspects of the life of people with disabilities. The study led to the conclusion that the current system of organizing the rehabilitation of people with disabilities needs to be improved, taking into account increased attention to the quality of the provision of such services and expanding the very concept of «rehabilitation» by including a professional, psychological and social component in it.

The work is devoted to the consideration of digital technologies as a tool that transformed entrepreneurship and helped save many new and established enterprises from bankruptcy during the spread of the COVID-19 pandemic. The practice of entrepreneurship in the Kazan period has shown that digital transformation allows to increase productivity and innovation in business models, value creation processes and interaction with customers and partners. In fact, digital entrepreneurship as a consideration of how digital technologies shape entrepreneurial activity seems to be blurring traditional entrepreneurial boundaries and expanding the entrepreneur's opportunities in the market, democratizing the ability to start, adapt or wind down enterprises.

PROBLEMS OF BRINGING TO CIVIL AND CRIMINAL LIABILITY FOR IMPROPER PROVISION OF MEDICAL SERVICES

Abstract
The purpose of this work is to consider the problems arising from the improper provision of medical services and bringing to civil and criminal liability for these actions of medical organizations and medical workers. The author investigates the issues of bringing to responsibility for violations related to improper provision of medical care in medical organizations of the healthcare system of the Russian Federation. Russian legislation enshrines the right of a patient to complain about the improper provision of medical services, both when applying to a state medical organization and when receiving medical care in private structures of the healthcare system. The problem of bringing to civil and criminal liability for improper provision of medical care in Russian healthcare has been standing for quite a long time, nevertheless, today it is coming to the fore, despite attempts to improve the legislative regulation of this issue. In conclusion, authors conclude that in order to achieve high quality medical services, it is necessary not only to comply with the requirements for the contractor, provided by licensing of medical activities and accreditation of medical specialists, but also to establish requirements for the activities of medical workers themselves. Thus, the provision of medical services should be considered improper if they are provided in violation of the requirements imposed on the subject and are provided in an improper way, i.e. in violation of the requirements that must be met by medical workers.

FEATURES OF THE STATE OF HEALTH, INCLUDING PHYSICAL DEVELOPMENT, OF ADOLESCENT GIRLS LIVING IN THE MIDDLE AND HIGH MOUNTAINS OF THE KYRGYZ REPUBLIC

Abstract
Adolescent girls aged 15—18 are in an active process of growth and development, as evidenced by the dynamics of changes in height and weight indicators. Low values of average growth indicators were determined among girls in Batken, on average 2 cm less than the average indicators of girls in Bishkek, however, the statistical analysis performed did not confirm the reliability of these differences. In 84.3% of the surveyed adolescent girls living in Naryn, iron deficiency anemia was detected. Kidney and urinary tract diseases (31.4%) took the first place in the structure of diseases among girls in Batken. Diffuse enlargement of the thyroid gland was found in 13% of women living in highlands. Inflammatory diseases of the genitourinary system are two or more times more common among girls in the middle and high mountains than in Bishkek: vulvitis in 8.7—11.4% of cases against 4.1% of cases, urogenital diseases in 6.5—17.4 versus 3.9 respectively. This indicates the need to introduce specific preventive programs for girls living in the middle and high mountains.

Abstract
In modern conditions of global threats caused by infectious risks, the importance of programs and preventive examinations of the working-age population is increasing, which make it possible to identify groups of people at increased risk in the formation of health disorders, and to obtain objective data on the dynamics of morbidity and mortality of the population, to conduct long-term monitoring. The uniqueness of this project lies in the fact that it is based on a patient-centered approach that involves the individualization of the patient's diagnosis, professional counseling about risk factors and a healthy lifestyle, accompanying the patient until the completion of the examination, diagnosis and treatment. The organization of preventive examinations of the population within the framework of the Healthy Moscow program was the result of coordinated work and the realization of the professional potential of various services in the healthcare system, education, science and public administration. The successful experience of implementing the Healthy Moscow program can be scaled up to other cities and regions of Russia, which will contribute to the development of the country's healthcare system and improve the quality of life.

A NEW APPROACH TO THE EPIDEMIOLOGICAL CONTROL OF INFECTIOUS DISEASES IN THE RUSSIAN POPULATION

Abstract
The article describes a development related to the field of public health and molecular biology, in particular, medical diagnostics. We present the Isascreen-8 portable analyzer (Registration Certificate No. RZN 2022/17322, May 24, 2022) and its application for rapid diagnostics of a new coronavirus infection with the use of commercially available sets IsoAmp SARS-CoV-2 (Lytech Co. Ltd., Russia), and also our own development TB-IZATEST (Invention Application No. 2022133809, December 22, 2022) for diagnostics of tuberculosis pathogen. The approach proposed in this article can be used for rapid detection of genetic material (DNA/RNA) of Mycobacterium tuberculosis and SARS-CoV-2 in biological samples using the method of loop isothermal amplification in less than 20 minutes, and also as a universal platform for detection of genetic material of other pathogens in a biological sample. Using the proposed approach, various research tasks for epidemiological monitoring of tuberculosis and new coronavirus infection, qualitative and quantitative analysis of bacterial and viral load in samples, including evaluation of the effectiveness of the prescribed antibiotic therapy regimen, can be solved. The advantage is that this method can be used not only in diagnostic laboratories, but also in field laboratories in the most remote regions of Russia if the mandatory sanitary and epidemiological requirements are observed. With sufficient distribution, the proposed approach could help ensure epidemiological control of the prevalence of these socially significant infections.

Abstract
The aging of the population poses to society the problem of finding mechanisms and tools for preserving health, prolonging the active life position of elderly citizens. The article reveals the potential of volunteerism in the system of strengthening the mental health of the elderly. The data obtained in the course of the study indicate that elderly citizens who participate in volunteer activities for a long time and regularly have high and average indicators of resilience, the presence of goals and the meaning of life, evaluate their lives as emotionally saturated, productive, the ability of elderly people to maintain psychological stability in stressful situations and orientation to overcome life difficulties.

Abstract
Anthropometric studies conducted in 1884, 1928, 1946, 1965 and 1999 to study the characteristics of growth and weight indicators of children living in the territory of modern Chuvashia, partly reflected the influence of the living conditions of children in certain historical periods of the country's development: revolutions, civil and domestic wars, post-war stabilization, «perestroika». Over the last 22-year period, from 1999 to 2021, there has been the largest increase in the growth rate of indicators of the physical development of children and adolescents, which is considered as a positive trend, a manifestation of ongoing acceleration processes. It was revealed that in modern children, physiological growth jumps in comparison with their peers in 1884, 1930, and 1999 occur earlier — in the prepubertal period.

Abstract
One of the directions to increase the effectiveness of medical organizations in the region is the development of managerial tools for improving the quality of medical services. The aim of the study is to develop an approach to improving the quality of medical services based on the results of measuring patient satisfaction with the quality of medical services, mapping satisfaction criteria to medical and technological characteristics of the quality of services, substantiation and selection of appropriate corrective measures. The method of measuring patient satisfaction with the quality of the provided medical service and the method of deploying quality functions ensuring transformation of patient needs and expectations into characteristics of service quality were used. The measurement of patients' satisfaction with the quality of medical services is carried out during medical and sociological research. The measurement results are obtained by statistical processing using a software set. To determine measures to improve the quality of cardiological services, the results of changing patient satisfaction were used in accordance with the method of deploying quality functions, which consists in implementing a number of steps that ensure the determination of a minimum set of additional requirements for medical services in order to meet the requirements of patients. This technique is supported by specialized software. Implementation of the proposed approach makes it possible to form tools aimed at creating an information model of medical services and ensure the change in the quality of such services demanded by patients.

Abstract
Purpose of the study: to analyze the differences in the normative (reference) values of the body length indicators of children and adolescents living in four ecological and geographical zones of the Republic of Sakha (Yakutia). Materials and methods. Statistical data processing was carried out on 24,123 schoolchildren who do not have chronic diseases belonging to health groups I and II. Statistical, analytical, mathematical and sociological methods were used. Results. Among boys, the highest level of statistically significant differences in body length was observed in the age groups of 12, 13 and 15—17 years, among girls — in the age groups of 13, 14 and 16 years. Higher reference values of body length indicators for boys and girls living in the industrial regions of the Republic of Sakha (Yakutia), lower — for those living in the Arctic zone of the Far North and rural areas of the Republic of Sakha (Yakutia), where the population is represented mainly by the indigenous peoples of the Far North.

The combined effect of close contact between child patients and otorhinolaryngologists, as well as the practice of aerosol-generating procedures, justify the need to take prompt organizational measures to reduce risks in the provision of pediatric otorhinolaryngological care in conditions of increased biological risks. In the pediatric ENT community, these adaptations have resulted in: judicious distribution of care to treat the most severe conditions, minimizing the risk of long-term consequences, ensuring the safety of child patients and healthcare workers through better communication methods, changing procedural precautions, the rapid introduction of telemedicine for diagnosis and treatment of patients, as well as the development of protective protocols to limit exposure to aerosol-generating procedures or manipulations to reduce the risk of exposure to a high viral load.

Abstract
The COVID-19 pandemic has spurred the introduction of new devices and technologies into the organization of otorhinolaryngological care, the development of safety measures and clinical work protocols in order to increase the safety of doctors and patients. Virtual outpatient clinics have been integrated into the pediatric and basic outpatient services, the potential of which for pediatric otolaryngology is considered quite significant. In otorhinolaryngological practice, the transition to the expanded use of telemedicine technologies (individual gadgets for examining ENT patients, including a direct access otoscope connected to an otolaryngologist’s computer) was especially significant. The practice of pediatric ENT services tends to combine virtual and face-to-face visits, even after the acute phase of the COVID-19 pandemic. When organizing otorhinolaryngological care, it is necessary to pay special attention to dialogic consensus — an inclusive and reflective dialogue between a doctor and a patient.

The most relevant research studies the role of immunity and microorganisms in the pathogenesis of rosacea. To elucidate the features of the comorbidity of rosacea and somatic diseases, studies in various populations with detailed clinical data and a longer follow up period are important. At the same time, it is important to understand the relationship between the severity and severity of various clinical symptoms of rosacea, the mechanisms that regulate the progression of the process, the combination of rosacea subtypes and the course of the disease in association with other forms of pathological disorders.

PERSONALIZED APPROACH IN THE DIAGNOSTICS AND TREATMENT OF SYMPTOMATIC EPILEPSY WITH DÉJÀ VU SEIZURES

Abstract
The phenomenon of déjà vu (DV) is of interest, occurring in 97% of healthy individuals, while DV can be a manifestation of a number of pathologies, such as Charles Bonnet syndrome, depression, schizophrenia, or temporal lobe epilepsy. Most cases of DV type epiphenomena are associated with hippocampal sclerosis, while up to 90% of patients with mesial temporal sclerosis are drug resistant. Despite the success of pharmacotherapy, the frequency of «uncontrolled» epilepsy in industrialized countries that adhere to modern standards of treatment is from 30 to 40%, and this percentage is higher among patients with symptomatic and cryptogenic forms than among patients with idiopathic epilepsy (respectively: 40% and 26%). In turn, when studying the DV phenomenon and choosing a therapy strategy, it is necessary to establish its origin, determine its clinical significance (whether it is initially pathological or not), and the need for treatment. During the analysis of exome data, a search was made for substitutions in genes associated with arteriovenous malformations, both with autosomal dominant and autosomal recessive types of inheritance. The genes KRIT1, RASA1, IL6, FAM58A, GLML, EPHB4, CCM2, and ELMO2 were analyzed especially carefully. The analysis of genetic data is of great importance in the aspect of preventing cerebrovascular accidents, at the same time, in order to obtain reliable and significant results, in addition to time and financial costs, examination of relatives is also required. Meanwhile, this fact does not mean that every patient needs to conduct a genetic study. The paper presents detailed instructions for supplementing anamnestic information, as well as the results of personalized instrumental and laboratory diagnostics, which made it possible to carry out timely correction of therapy and achieve a prolonged positive effect.

BRIEF DESCRIPTION OF THE PERSONNEL POTENTIAL OF HEALTHCARE IN GREECE, SPAIN AND BULGARIA

Abstract
The article presents a brief descriptive overview of the human resources of healthcare in Greece, Spain and Bulgaria. It is noted that they differ in the redundancy of specialist doctors with a relative shortage of general practitioners. The medical personnel of these countries are characterized by aging, a tendency to migrate to other countries, a shortage of secondary medical personnel, an excess of the required number of dentists, which is especially pronounced in Bulgaria. In addition, the availability of medical care and the limits on the number of patients for a certain time period with one doctor, established in Greece, reduce. The salary level of doctors in these countries varies from the minimum (Greece, Bulgaria) to the maximum in Spain. At the same time, the training of medical personnel is carried out according to the Bologna system (bachelor's degree, master's degree, etc.) and is characterized by a long period (4—5 years) of preparing a doctor for independent work.

REVIEW OF FOREIGN HEALTHCARE SYSTEMS AND ONGOING REFORMS IN CHINA AND VIETNAM

Abstract
The article is devoted to the review of some aspects of health care reform abroad, the emphasis is placed on the study of measures to achieve universal health coverage (HEU) in China and Vietnam. There is a significant similarity of problematic issues in the health care of most countries, which include ensuring equal access to quality medical care, shortage of medical personnel, financing of medical care. A review of the studied reforms showed that those that are addressed to a specific health problem achieve greater success, especially in the field of public health reform. The experience of reforms in China and Vietnam in the field of increasing the availability of medical care demonstrates the importance of improving the financing of the medical industry in the implementation of most reforms. Both countries have implemented reforms in the field of health insurance, but in China basic health insurance was introduced, while in Vietnam more attention was paid to vulnerable groups of the population. Despite the fact that the two countries have made progress in health coverage, inequality in access to medical care has been significantly reduced in Vietnam. It is concluded that the effectiveness of reforms will be more successful if such measures as the use of strategic procurement, improvement of primary health care, improvement of the quality of services, provision of political support and increased financing of health care are implemented.

BRIEF DESCRIPTION OF SOME ASPECTS OF PUBLIC HEALTH IN AUSTRIA, BELGIUM, GERMANY AND ESTONIA

Abstract
The article, which reviews some aspects of public health in Austria, Belgium, Germany and Estonia, highlights the achievements of public health in these countries, which is expressed in an increase in life expectancy, primarily due to a reduction in mortality from cardiovascular diseases. In the countries listed above, there is a difference in life expectancy, both between men and women, and between people with higher and basic education. There is still an urgent problem of the prevalence of risk factors — smoking and alcohol consumption, while the level of physical activity of residents of the designated countries can be called high. The fight against risk factors in these States is carried out, among other things, through the implementation of screening programs. The digital transformation in healthcare in Austria, Belgium, Germany and Estonia is also intensifying. A significant part of medical data, including informational and educational materials, is now available to the public in digital format.

SOME ASPECTS OF ENSURING PATIENTS' RIGHTS AND ACCESSIBILITY OF MEDICAL CARE IN AUSTRIA, BELGIUM, GERMANY AND ESTONIA

Abstract
The article devoted to ensuring the rights of patients and the availability of medical care in Austria, Belgium, Germany and Estonia notes the experience of creating an institute of independent ombudsmen who plays an important role in this issue. Basically, in these countries, the choice of a medical organization and a doctor is possible, and in Belgium the right to choose an insurance organization is granted. Patients in these countries are given the right to control the process of providing them with medical care, including its payment and joint decision-making with the doctor on treatment tactics. The State also ensures the patient's right to receive preventive and restorative services provided to children free of charge, and compensated for adults in 80% of cases. It is impossible not to recognize the high percentage of citizens of these countries, reaching 98%, satisfied with the medical services provided to them. However, in some countries there is an imbalance in the availability of medical care, which is expressed by high availability of family doctor services and low availability of specialist services, as in Estonia. Nevertheless, this does not cause an increase in the level of corruption in the medical spheres of these countries, which is below the European average. According to the totality of indicators of the availability of medical care, Germany is the leader among the analyzed countries.

Abstract
The article provides a brief overview of the current problems of health systems in a number of European countries and measures aimed at solving them. Noting the high level of health systems in these countries, it is also necessary to recognize their expensive cost due to the predominance of inpatient medical care. Almost all health care systems are experiencing staff shortages of varying degrees of severity, difficulties with ensuring equal access to medical care. Understanding the existing problems leads the governments of a number of countries, such as Austria, to formulate the goals they want to achieve in healthcare and monitor them. Among the measures taken, it is worth noting innovations in healthcare financing, the granting of additional powers to inpatient medical organizations, and others.

BRIEF DESCRIPTION OF CURRENT PROBLEMS AND ONGOING REFORMS OF HEALTHCARE SYSTEMS IN GREECE, BULGARIA AND SPAIN

Abstract
The article is a description of the problems and reforms of the healthcare systems of Greece, Bulgaria, Spain. The range of existing problems includes the imperfection of management and financing of healthcare in these countries, which leads to a high level of co-financing of medical care by the population itself, a shortage of medical personnel and their uneven distribution within countries, the predominance of inpatient medical services, reduced availability (waiting time for medical care). In addition, Greece and Bulgaria have a high prevalence of risk factors for non-communicable diseases with weak or no screening programs. The countries listed above are implementing a number of reforms related to the management and financing of healthcare, and the availability of medical care. However, significant results of reform, with the exception of Spain, are not observed in these countries, including due to the lack of political agreement on the implementation of the necessary reforms.

Abstract
The article devoted to the description of some aspects of inpatient care for the population of a number of European countries presents data on the number of hospitalizations, provision of inpatient beds for the population, financing of inpatient services. There are different approaches in the organization of inpatient care and its management, low rates of occupancy of inpatient beds are recorded, which indicates an oversupply of medical services. As an interesting experience, the facts of attracting trained volunteers in Austria to provide emergency care are given. It is noted that it is necessary to continue to reorient the provision of medical care from the inpatient sector to the outpatient one.

BRIEF DESCRIPTION OF THE HEALTHCARE SYSTEMS OF GREECE, SPAIN AND BULGARIA

Abstract
The review of healthcare systems presented in the article notes the diversity of healthcare management in Greece, Spain, and Bulgaria. All these systems have more or less pronounced state subsystems with different centralization of management, the maximum level of which is represented in Bulgaria. Medical care is provided at several levels, while the provision of primary health care still requires improvement in Greece and Bulgaria. A fairly high percentage of GDP spent in these countries on healthcare does not prevent the co-financing of medical services at the expense of personal funds of citizens, including through informal payments. At the same time, in all three countries, the provision of medical care in hospitals still prevails over outpatient care.

The main personal and professional qualities of the doctor, which are associated with the concept of «professionalism» in patients, are noted. It is noted that the formation of professional behavior is influenced not only by the personal qualities of a medical worker, but also by the working environment of the medical organization in which he carries out professional activities. SYMBOLISM OF THE WHITE COAT IN THE MEDICAL PROFESSION

Abstract
It has long been a tradition for doctors to dress professionally in white coats — a universal symbol of belonging to the medical profession. This tradition dates back to the time of Hippocrates, but during the XIX—XXI centuries, the symbolism of the white coat was criticized in connection with research on the spread of infections through tissue. Currently, many doctors refuse to use the oldest symbol of the profession, and the practice of medical activity is replete with new variants of the uniform of a medical worker (both in style and in color palette. However, it should be noted that the white coat symbolizes another important part of the medical education of students, the standard of professionalism and care, as well as a symbol of the trust they must earn from patients.
