THE EVALUATION OF ACCESSIBILITY OF MEDICAL CARE OF RURAL RESIDENTS

Abstract
The article considers dynamics of indices and indicators characterizing accessibility of medical care to rural population. The dynamic analysis of the period before and after implementation of the National Projects of Russia “Health”, “Education”, “Housing” and “Agriculture” was used as basis of assessing accessibility of medical care to rural residents. The research for the period 2014–2023 revealed that during implementation of the National Projects there was significant improvement in quality of structure of medical care of rural residents. However, problem of staffing shortage remains unsolved since increasing of staffing with physicians that is largely conditioned by decreasing of number of medical organizations, continues to further decrease indicators of provision of rural health care with medical personnel. Moreover, assessment of accessibility of medical care in hospitals demonstrated decreasing of level of number of hospital beds in rural areas and rate of hospitalization of rural population. The study revealed that in all Federal Okrugs of the Russian Federation there are problems of accessibility of medical care to rural population. Despite positive dynamics related to implementation of the state programs, conclusion is made that it is necessary to increase accessibility of medical care to rural population through both improvement of traveling forms of medical care in remote settlements and increasing of medical activity of rural residents.

THE MODELS OF BEHAVIOR OF PATIENTS UNDER INTERACTION WITH HEALTH CARE SYSTEM AS EXEMPLIFIED BY DIGESTIVE APPARATUS DISEASES

Abstract
The role of patient, especially at early stages of disease, is decisive one in relation to disease outcome. Therefore, it is extremely important to analyze typical and deviant forms of one's behavior in interaction with health care system. The purpose of the study is to analyze models of behavior of patients with diseases of digestive system in interaction with the health care system. Three comparison groups were formed for analysis. The first one included patients undergoing planned out-patient treatment (n=531), the second one — patients undergoing planned in-patient treatment (specialized Gastroenterology Department) (n=822). The third one — patients admitted for in-patient treatment at the Emergency Medical Care Hospital on emergency basis (n=147) on account of complications of primary disease (pathology of digestive organs). The groups were compared using such non-parametric criteria as the association coefficient (Cass) and the χ2 criterion. It is established that at choosing physician, the most frequently used source of information by patients is opinions of ones who have already was treated by particular physician. In all three groups, this source of information was used most frequently. The second source most frequently used was one's own previous experience of visiting this physician. The third most important criterion of choice was patient own opinion. At that, no reliable differences in using of information sources by patients included in first and second groups were found. The significant differences in behavior models were found among patients of third group that significantly affects possibility of complications requiring emergency hospitalization. The main criterion for choosing medical organization, if such an opportunity exists, is availability of competent specialists that is considered much more important than good reputation of medical organization. The geographical accessibility is also of great importance for patients, especially for ones receiving medical care in out-patient conditions. The specific characteristics of the models of behavior patterns of patients in each group should to determine other general characteristics of behavior associated with interaction of patient and the health care system.

Abstract
The article considers becoming and development of concept of active longevity in the state demographic policy of the countries of the Commonwealth of Independent States (CIS). On the basis of analysis of program documents, strategies and institutional mechanisms of the CIS member states, common characteristics and national peculiarities of implementation of policy related to the aging population are identified. The special attention is paid to issues of institutionalization of approach to active longevity, as well as to role of interstate cooperation in promoting age-friendly agenda.

THE REVIEW OF SANCTION MEASURES OF UNFRIENDLY COUNTRIES RELATED TO EXPORTING AND RE-EXPORTING OF MEDICAL ITEMS TO RUSSIA

Abstract
The constantly increasing measures of sanctions impact contribute to advancing development of political conflict of Russia with unfriendly countries, that results in increasing economic pressure on sectors of the Russian consumer market that are the most import-dependent ones. Currently, the European Union, the United States, Japan and other states that received status of unfriendly ones, are actively applying secondary sanctions originally developed as tool to curb schemes of circumventing existing bans against Russia, of which legalization of parallel imports, introduced in 2022, was among the most effective ones. By their content, secondary sanctions imply possibility of imposing prohibitive measures on foreign counter-agents from countries that are friendly for Russia, that cause additional difficulties with registration, making payments and increased delivery time. Of course, all the circumstances mentioned above could not help but affect export and re-export of medical devices to Russia. The detailed analysis of content of imposed sanctions measures established that currently there are no direct bans on export and re-export of medical equipment, as well as components and consumables, parts and accessories. The only exception are medical devices classified as dual-use goods and fairly large number of complex technological equipment, including its components, that fell under measures of strengthening export control imposed by the United States. Despite the fact that there are no formal sanctions against medical equipment and articles, in practice they are actively used, and given absence of unified methodology, issue of their implementation becomes even more ambiguous and problematic that makes the topic under consideration relevant. The article examines in detail main content of the prohibitive and restrictive measures taken within the framework of the sanctions impact from unfriendly states in relation to export and re-export of specialized medical equipment and components. Also are considered the listing of goods classified as of dual-use in terms of medical devices. The detailed list of medical products for which the US requirement to issue an export license for their export applies is presented. The practical recommendations are formulated to address main problematic issues.

THE LEGAL MEANS OF PROTECTING MEDICAL WORKERS FROM UNFOUNDED ACCUSATIONS (CLAIMS) FROM PATIENTS (PROCEDURAL ASPECT)

Abstract
The medical workers carrying out medical activities come into focus of significant number of interested participants of complex social and public relations. The medical activity is an important component of preserving life and health of society and has complex nature, is subjected to various quality assessments and forms space for polemics and expression of subjective opinions. If such assessments do not form offensive or defamatory content, then even being not loyal in message, they can be expressed without any restrictions, using all permitted resources, including mass media or Internet system. Otherwise, medical workers are given the right to protect one's honor, dignity and business reputation through various methods and means provided for by law. The purpose of the study is to analyze practical aspects of protection of business reputation with different composition of participants and diverse options for decisions taken by judicial authorities. The reflection in materials of judicial practice of problematic situations occurred from the fact of seeking medical care and subsequent assessment of physician's actions as public subject of social activity permitted to conclude that such intangible benefits as good name, business reputation and private life form basis of personality and are subjected to protection and defense on equal basis with other benefits recognized by society.

THE ANALYSIS OF ACTIVITIES ON INCREASING EFFICIENCY OF ANTITUMORAL MEDICINAL THERAPY, SURGICAL AND RADIATION TREATMENT OF PATIENTS WITH ONCOLOGICAL DISEASES

Abstract
The article considers system of measures improving efficiency of anti-tumor medicinal therapy, surgical and radiation treatment of patients with oncological diseases, including development of proposals for their improvement. The sampling included 485 experts (practicing oncologists). The expert survey was carried out using expert questionnaire to develop proposals improving organization of medical care of patients with oncological diseases. The Kendall's concordance coefficient (W) was calculated to quantify degree of concordance of answers of experts. The significance of differences was assessed by non-parametric Friedman criterion. The reliability and validity of questionnaire scale was carried out by calculating the Cronbach's alpha coefficient. The results demonstrated that in order to formulate proposals improving organization of oncological medical care management, current indicators and implemented measures of organization of medical care of oncological patients are be taken into account, including percentage of patients with oncological diseases, who underwent examination and/or treatment during current year out of those under dispensary observation, the percentage of malignant neoplasms detected at stages I-II and organization of reference centers that use histological, immunohistochemical, pathomorphological and Xray methods of research. The following indicators are recommended to apply for improving organization of cancer medical care: control compliance of drug therapy with clinical recommendations; increasing surgical activity and availability of surgical interventions by increasing volume of medical care; adding new results and outputs targeted to increase the number of molecular researches when prescribing targeted anticancer drugs; centralization of the cancer medical service. The indicators, that can be used to formulate and prioritize proposals for improving organization of cancer medical care, and to form Federal projects or departmental special-purpose program, were selected. The results of the study can be used in development of system to monitor efficiency of implementation of these projects and programs.

THE MANAGEMENT MODEL FOR EVALUATING EFFICIENCY OF ACTIVITIES OF MATERNITY OBSTETRIC ORGANIZATIONS OF THE TVER OBLAST IN DECREASING MATERNAL AND INFANT MORTALITY

Abstract
The article presents management model to assess potential of sustainable development of medical organizations. The model can be used in assessing efficiency of activities of maternity organizations in the Tver Oblast to reduce maternal and infant mortality. The basis for implementation of the model is analysis of data from reporting documentation on functioning of maternity institutions in the Tver Oblast. It was established that with implementation of the management model, the manager can set in motion the leadership potential and to evaluate efficiency of innovations even before they start working in the institution.The decreasing of maternal and infant mortality was noted at the expense of introduction of new techniques. The introduction of pediatric component is justified for further decrease of infant mortality.

THE RISK AND PROTECTIVE FACTORS FOR NONCOMMUNICABLE DISEASES

Abstract
The article presents modern data on non-communicable diseases (NCDs) and their risk factors. The tendency of increasing mortality from NCDs due to increase in the number and aging of the population is noted. The data is presented testifying that, in addition to biological risk factors, influence of environmental factors is increasing, including atmospheric and household air pollution, the levels of which are high in a significant number of countries. It is emphasized that “green spaces” (e.g., trees, grass, forests and parks) and “blue spaces” (e.g., lakes, rivers, ponds, etc.) provide beneficial effect on human health, being in fact protective factors for NCDs. The NCD risk factors are found to interact closely with each other: air pollution, depression, tobacco smoking, high blood pressure and obesity have been linked to all NCDs. The evidence is provided that presence of behavioral risk factors for NCDs exacerbates severity of infectious pathology, particularly COVID-19 and other infectious diseases (influenza, HIV, tuberculosis, hepatitis, etc.). Given that NCD risk factors can occur in early childhood, promoting healthy lifestyles among expectant mothers and adolescents should be a priority to reduce NCD risks, especially in low- and middle-income countries.

THE STUDY OF PREVALENCE OF MAIN RISK FACTORS OF DEVELOPMENT OF CHRONIC INFECTIOUS DISEASES IN RESIDENTS OF MAKHACHKALA

Abstract
In Russia, mortality from major non-communicable diseases accounts for more than 2/3 of total mortality of population. The purpose of the study is to investigate prevalence of main risk factors of development of chronic non-communicable diseases (NCDs) among patients. The study was carried out in 2023–2024 at the basis of the Republican Health Center of Makhachkala of the Republic of Dagestan. The prevalence of risk factors for non-communicable diseases was analyzed using results of sociological survey of 465 respondents and outpatient card data. The low physical activity, high cholesterol, overweight, obesity and high blood pressure are more common among persons with higher education. The smoking, excessive salt intake and insufficient consumption of fruits and vegetables are more common among persons with secondary and secondary specialized education. The highest percentage of smokers was detected in groups of 41–50 age old (24.7%) and 31–40 years old (20.7%). The insufficient awareness about one's level of cholesterol was detected in 17.8% of males and 23.9% of females and about one's level of blood sugar in 34.6% of males and 38.1% of females. The prevalence of main risk factors for NCDs was established for high blood pressure — 30.1%, overweight — 29.2%, obesity — 14.9%, smoking — 18.1%, elevated cholesterol — 41.5%. This data is to be taken into account in developing regional educational preventive programs.

THE COMPARATIVE ANALYSIS OF MORTALITY FROM MALIGNANT NEOPLASMS IN MOSCOW AND THE MOSCOW OBLAST

Abstract
The City of Moscow and the Moscow Oblast form the Moscow agglomeration, but urban residents have advantages over rural ones. The contribution of deaths from malignant neoplasms (MNP) into mortality from all causes in higher in Moscow (23.3±0.5% and 18.3±2.8%) than in the Moscow Oblast (15.0±0.3% and 12.3±2.2%). The standardized mortality rate from MNP in Moscow made up to 146.82±2.05 per 100,000 of population in 2017–2019 and 131.35±8.92 in 2020–2022. In the Moscow Oblast the mentioned indicator made up to 138.54±0.12 and 131.84±4.10, respectively. In both City of Moscow and the Moscow Oblast 80% of deaths from all MNP in 2017–2019 and 2020–2022 were caused by 14 out of 33 groups with MNP and 13 of them coincided in both Subjects of the Russian Federation. The percentage of MNP of five localizations (trachea/bronchi/lungs, colorectal cancer, stomach, breast and pancreas) in Moscow made up to 50.6% and 51.7%, and in the Moscow Oblast up to 55.1% and 55.2% in both time periods accordingly. The COVID-19 pandemic resulted not in increasing of standardized mortality rate from MNP, but in decreasing of its percentage in the structure of total mortality. Despite differences between Moscow and the Moscow Oblast, the mortality from MNP in both Subjects of the Russian Federation differs insignificantly, and structure of causes of mortality from MNP is factually the same. The obtained data is an additional basis to confirm sufficient efficiency of the system of organization of medical care of patients with MNP in the studied Subjects of the Russian Federation.

THE UROLITHIASIS IN MEMBERS OF SEAGOING CRAFT CREW: PROFESSIONAL RISKS, PREVENTION AND MEDICAL SUPPORT

Abstract
The work of crew members of sea crafts is associated with numerous occupational risk factors that can contribute to development of chronic diseases, including urolithiasis. This article considers main causes and mechanisms of development of urolithiasis among seamen, conditioned by characteristics of their professional activity: unbalanced diet, use of distilled water, physical loads, prolonged confinement in limited spaces and stress factors. The statistics of prevalence of urolithiasis in Russia and globally are presented. The complicacy of diagnostics and provision of medical care aboard ships is analyzed too. The special attention is paid to such preventive measures as diet management, maintenance water intake schedule, stress management and dispensary monitoring.The approaches to metaphylaxis and role of sanatorium-resort therapy in prevention of disease relapses are discussed. The conclusion is made about importance of comprehensive approach to prevention and treatment of urolithiasis among seamen as significant medical and social problem.

THE MAINTENANCE OF PSYCHOLOGICAL HEALTH OF FEMALE SAILORS IN MERCHANT SHIPPING

Abstract
Many shipping companies encourage women to be represented in maritime industry by offering them training and support programs. However, research studies confirm that low percentage of presence of women on ships is related to hard and dangerous work negatively affecting health and psychological condition. The results of original survey demonstrated that specifics of working on ship and lack of communication with relatives have extremely negative effect on emotional state of female crew members. However, it is established that all things being equal, women are more at risk of development of depressive condition as compared to men, which is conditioned by both physiological and social cultural factors. The main causes of stress and depression among female seamen are considered. The impact of stressful situations on human body are considered. The recommendations targeted to preserving psychological health of female seamen in order to ensure comfortable working conditions on board are proposed.

THE MAINTENANCE OF TECHNICAL REGULATION OF ACTIVITIES OF CUSTOM AUTHORITIES AT IMPORT OF ELECTRONIC CIGARETTES AS MEASURE OF PROTECTION OF LIFE AND HEALTH OF CITIZEN

Abstract
Since 2006, a world trend began to develop concerning smoking of various electronic cigarettes, which gained enormous popularity due positioning by manufacturers as “safe” alternative to traditional cigarettes, combined with attractive design and many variations of flavor additives. The electronic cigarettes and their components referred to nicotine-containing products are imported to Russia from other countries. The most of this group of goods is counterfeit. The component composition of liquids and tobacco mixtures used in electronic cigarettes demonstrates presence of admixtures and substances that are not safe for human life and health and can ruinously affect cardiovascular and respiratory systems, mucous membrane of colon, nervous, endocrine and reproductive systems. The research study permitted to identify shortcomings in current legislation concerning import of electronic cigarettes to Russia, in terms of compliance with technical regulation measures in order to ensure safety of life and health of citizens. The various modifications of electronic cigarettes are considered. The analysis of normative legal documents regulating circulation of nicotine-containing production on territory of Russia was implemented. The shortcomings in legislation are established. The appropriate recommendations for their elimination are formulated.

THE ART-THERAPY AS HEALTH-PRESERVING TECHNOLOGY AT WORK WITH CHILDREN FROM PROBLEM FAMILIES

Abstract
The considered issue is conditioned by need in studying psycho-social problems of children from problem families and searching for technologies to resolve them. The article is devoted to study of health-saving technologies of art therapy applied to children from problem families. It is established that psycho-social problems are widespread among children and adolescents. According to WHO data, their prevalence worldwide is estimated at 10–20%. These problems can seriously interfere with daily life and increase risk of poor academic performance at school. The study revealed emotional (anxiety, depressive feelings, withdrawn behavior, psychosomatic complaints), behavioral (hyperactivity, aggressive behavior, behavior problems) and social (social isolation, lack of social contacts and interactions) problems of children. It is determined that art therapy technology is used to increase self-esteem and self-awareness, to develop emotional stability, to improve social skills and activity and to reduce stress. In recent years, art therapy, as socio-psychological technology, played important role in solving whole range of tasks to preserve the psycho-social health of children, especially ones from problem families. This method is associated with disclosure of personal creative potential, manifestation of social activity, release of one's hidden energy reserves and, as a result, finding optimal ways to resolve one's problems. It is concluded that application of health-saving art therapy technology can provide children from problem groups of population with opportunity to resolve psycho-social problems through “self-healing”, reducing level of social tension, increasing level of emotional well-being.

THE MEDICAL SOCIAL AND PSYCHOLOGICAL PROBLEMS OF PATIENTS WITH DIAGNOSIS OF RECTUM CANCER DURING POSTSURGERY PERIOD: THE PUBLICATIONS REVIEW

Abstract
The review is devoted to medical social problems of early and late postoperative period in patients with malignant tumors of rectum. the analysis of publications provided information testifying that from the first days after surgery, adaptation difficulties affect not only patient, but also one's family. the physiological difficulties in adapting to new living conditions are associated not only with such general negative factors as traumatic nature of operation, blood loss and operational stress, but also with peculiarities of new anatomical status of gastrointestinal tract. The presence of colostomy, stool and gas incontinence often result in social isolation of patient at the level of both family and society. A number of ethical and deontological problems are noted in interaction of the operated patient with attending oncologist at inpatient and outpatient stages of treatment. It is concluded that only comprehensive and complex understanding of medical, psychological and social aspects of problems of the postoperative period can lead to development of programs for their effective solution.

NUTRITION MANAGEMENT FOR COGNITIVE DISORDERS AND DEMENTIA (OVERVIEW)

Abstract
The current trend of increasing life expectancy and aging of the world's population is accompanied by the emergence of a global problem associated with a doubling of patients with «senile dementia» every 20 years. The concept of age-associated forms of cognitive disorders (vascular dementia, Alzheimer's, Parkinson's, etc.) is based on the provisions of the effects of unhealthy lifestyle and nutrition habits, family history, involutive metabolic processes, pathological complications of diseasesand their comorbidity. A literature search of systematic reviews, clinical research reports on the publication type «meta-analysis» and in the Medline system revealed that timely correction of negative PH, in particular the use of herodietics, helps to reduce/slow down the development of vascular dementia and cognitive disorders.

THE INCLUSIVE EDUCATION IN HIGHER SCHOOL: PERSONALIZATION, ACCESSIBILITY, DIGITIZATION

Abstract
The relevance of inclusive education in higher education institution is determined not only by moral imperatives, but also by pragmatic needs of modern society. The modern labor market requires variety of talents and competencies. The inclusion forms conditions for disclosure of potential of those students who traditionally excluded from educational process. The article draw attention to such key issues of increasing inclusiveness of higher education as attention to social and social cultural components of socialization of the youth that university can provide. Many physical and information barriers that traditionally prevented the disabled to obtain higher education can be eliminated by digital technologies. The digitization erases geographical boundaries and creates equal opportunities for everyone, regardless of place of residence and health status. The specialized software, assistive devices, online platforms can significantly facilitate learning for students with disabilities. However, it is important not just to implement technologies, but to train students and teachers to apply them effectively.

THE ANALYSIS OF PSYCHOLOGICAL FACTORS IMPACTING ADAPTATION OF WORKERS OF POLYCLINICS TO ALTERATIONS IN WORK INCLUDING ONES RESULTED FROM IMPLEMENTATION OF PROJECT ACTIVITIES

Abstract
According to many criteria, implementation of lean technologies into medical organizations rendering primary health care is implemented through project activities. At that, experience of implementation of projects presents different results in terms of efficiency in clinics of the Moscow Oblast that is partly related to psychological characteristics of employees and unpreparedness for changes in work. The purpose of the study is to assess particular psychological characteristics of medical workers in primary health care organizations which may affect implementation of changes in basic processes in functioning of clinics. The questionnaire survey of 3 978 managers and workers of polyclinics of the Moscow Oblast was carried out concerning differential type of reflection, cognitive regulation of emotions and integral types of communicative aggressiveness to analyze psychological status of workers of polyclinics where project activities were implemented. According to differential type of reflection average level of systemic reflection is decreased in 62.6% of respondents. In 7% evidence of quasi-reflection indicator was revealed. In 20.78% of employees deviation in introspection parameter was detected. On cognitive regulation of emotions self-blame was above “norm” in 32.5% of respondents; indicator of rumination was above “norm” in 22.8% of workers, deviation from the “norm” in terms of catastrophizing was marked in 7.9%, blaming others in 4.5%, acceptance was decreased in 70.6% of respondents, positive refocusing was insufficient in 77.7% of workers of polyclinics, perspective consideration was insufficient in 65.8% of respondents; and 51.2% of respondents had weak focusing on planning. On integral types of communicative aggressiviness low level of communicative aggression was established. At that, in 85.9% of respondents parameter “inability to switch aggression to activities or inanimate objects” was above average value. To increase efficiency of results of implementation of changes within the framework of project activities it is necessary to carry out psychological work with personnel of medical organizations in individual and group (training) format.

Abstract
The article presents design of historical study of the Russian pharmaceutical market, setting timeline in the nineties — the time of formation of social and cultural characteristics of this phenomenon. At the same time, the authors return to the post-Soviet period and the nineties to see starting positions as well as mutations of original idea. The article is structured by the framework format of dissertation abstract. This frame contains the following elements: research questions, assessment of state of research field, working hypotheses, the author's vision of social and scientific significance of the study being conducted and observations made. The definition of the research genre as dystopia is explained by its deconstruction of the palimpsest of memory testimonies of pharmaceutical market actors.

THE CHARITABLE CAPITAL AT SOURCES OF ORGANIZATION OF MEDICAL SOCIAL CARE OF VISUALLY IMPAIRED CHILDREN OF THE EASTERN SIBERIA IN XIX CENTURY

Abstract
The article presents results of studying archival sources and periodicals that reveal history of development of medical social care to the blind in the Irkutsk Governorate-General. The blindness was especially common among residents of the northern districts of Eastern Siberia. The glaucoma, traumatic and inflammatory affections of the cornea were the causes of development of blindness in conditions of absence of specialized medical care. But much more often it was caused by trachoma, which became widespread. The events are displayed in chronological order. The organization of public institutions, role of charitable capital, eparchy bodies and local authorities in resolving financing issues, attraction of public attention to problem of blind children were examined. The leading role in this process played most various categories of philanthropists, from principal patrons of arts to ordinary physicians providing free medical care. The organization of flying ophthalmological squads became prototype of organization of specialized medical care. The role of guardianship movement of helping blind children in Eastern Siberia is demonstrated. The article presents history of organization of school for blind children in the main city of the Gubernia, founded with funds from testamentary capital of the widow of Ivan Stepanovich Khaminov. This school became the first such institution in the Eastern Siberia. The organization of the pedagogical, educational and labor process for blind children in the school is studied in detail.

Abstract
The alarming trend in modern epidemiology of atopic dermatitis is omnipresent steady increase in its prevalence making the problem relevant. The purpose of the study is to shed light on historical aspects of development of ideas about atopic dermatitis and differences in classification approach. The search of publications was carried out in such Internet databases as Scopus, MedLine, еLibrary, Google Scholar according keywords atopic dermatitis, pathophysiology, history, classification. The study of etiology and pathogenesis of atopic dermatitis at different levels took thousands years. The theories and concepts of atopy and allergy were developed. The IgE and its key role in development of immune mediated diseases was discovered. The term atopic dermatitis was introduced. including its diagnostic criteria and classification. The concomitant and unrelated diseases were identified. The definition of dermatitis and eczema as synonyms that can be used to describe picture of inflammatory reaction in skin, histologically characterized by spongiosis with different degrees of acanthosis and presence of superficial perivascular lymphohistiocytic infiltrate. The clinical signs (itching, redness, peeling and papulovesicles of the skin) are developed under influence of combination of endogenous and exogenous factors, characterized by recurrent course and worsen quality of life of patients. The dermatitis and eczema requires identical approaches to treatment and prevention. Over the millennia, many scientists, physicians and dermatological schools contributed into description of pathophysiology and clinic of this disease that is important to be studied in order to understand modern concept and classification.

THE INDICATORS OF FUNCTIONING OF EMERGENCY MEDICAL CARE IN HOSPITAL CONDITIONS IN THE RUSSIAN FEDERATION

Abstract
The emergency medical care is an important form of medical care of population which is applied outside medical organization, in in-patient and out-patient conditions. In the Russian Federation, over the last decade, modernization of functioning of emergency medical care in in-patient settings is in process, on the basis of organization of such new structural units as in-patient emergency medical care departments. The purpose of the study is to analyze indicators of performance of emergency medical care in in-patient conditions in the Russian Federation in 2014–2023. The normative legal documents approving organization of emergency medical care in in-patient conditions were studied. The analysis of indicators of performance of emergency medical care in in-patient conditions was was implemented according to the data of the Federal statistical observation form №30 in 2014–2023. The content analysis, analytical and statistical methods were applied. As a result of modernization of emergency medical care in in-patient conditions over last 10 years, an increase of indicators of provision of emergency medical care beds was established from 2.84 to 6.25 per 1 million of population, the rate of hospitalizations to emergency medical care beds — from 0.8 to 2.51 per 1000 of population, bed turnover — from 262 to 332 days, decrease in average duration of patient's stay in a bed — from 1.05 to 0.63 days, average bed occupancy per year — from 277 to 207 days.

THE POTENTIAL OF PUBLIC ASSOCIATIONS ON BUILDING UP SOCIAL HEALTH OF CITIZEN IN THE COUNTRIES OF THE EURASIAN ECONOMICAL UNION: SOCIOLOGICAL DEMOGRAPHIC ANALYSIS

Abstract
The article considers issue of expediency of discovering potential of national and international public associations on building up social health of citizens in the states members of the Eurasian Economical Union (EAEU). The problem is in absence of scientifically grounded sociological demographic approach to applying capabilities of public sector in building up people’s social health. The study is based on official statistics of the EAEU, as well as on the results of public opinion poll by VCIOM (Russian Public Opinion Research Center). The analysis of the qualitative side of interpersonal relationships permitted to conclude that people’s circle of friendly communications is mainly concentrated on close relatives, helping to strengthen social health of citizens. The following concept was proposed: since rail and air transport initially suppose spatial movement of people over longer distances than road transport, there is probably tendency of increasing of social communications between persons located at significant territorial distance from each other. It is established that further research of potential of national and international public associations can become an important tool of determining current social demographic trends and developing recommendations to improve social health of citizens of the EAEU member states.

Abstract
The social economic status of population evidently affects health. Actually, in National research studies wide enough spectrum of approaches to determining and choosing indicators of social economical status in health studies is applied. The purpose of the study is to investigate and systematize parameters applied in studies of health of of different groups of population of the Russian Federation. The methodology of Cochrane Rapid Reviews Methods Group was applied to implement rapid review. The express review was implemented on the results of publications searches in data bases of the National Electronic Library and the Electronic Scientific Library in “Health Sciences” and “Medicine and Health Care” domains from 2017 to 2023. The results demonstrate that the most widespread parameter of social economical status is “education” followed by “income”, “line of business” and “urban/rural residence locality”.

THE ANALYSIS OF COURSE OF PREGNANCY, DELIVERY AND CONDITION OF NEWBORNS IN WOMEN WITH GESTATIONAL DIABETES MELLITUS IN IVANOVO IN 2014–2022

Abstract
The gestational diabetes mellitus is common chronic disease complicating course of pregnancy and childbirth. The retrospective study included analysis of course of pregnancy, childbirth and condition of newborns of 2000 women with gestational diabetes mellitus observed in antenatal clinics in Ivanovo in 2014–2022. By 2022, rate of such complications and outcomes of pregnancy as early and late threatened miscarriage, vomiting of pregnant women, premature detachment of normally located placenta, placental insufficiency, pathological body weight gain, diabetic fetopathy, predelivery rupture of amniotic fluid, premature birth is decreasing. The condition of fetus according to the Apgar scale is improving. By 2022, in women with gestational diabetes mellitus such indicators as isthmian-cervical insufficiency, anemia, hypertensive disorders, fetal growth retardation, insufficient body weight gain during pregnancy, head double contour, clinically narrow pelvis in childbirth and congenital malformations of the fetus increased. The timely diagnosis of gestational diabetes mellitus and pregnancy complications, continuity of specialized medical care will permit to reduce rate of unfavorable pregnancy complications and to improve perinatal outcomes.

THE ISSUES OF INCREASING OF EMPLOYMENT LEVEL OF PERSONS WITH RESTRICTED POSSIBILITIES IN RUSSIA

Abstract
In the current period, Russia faces increasing shortage of workers in many sectors of economy. At that, employment rate among citizens with established disability group remains quite low as compared with developed countries that may testify presence of certain obstacles for their appearance at labor market. The The study contemplated to find out causes of low employment of this category of citizens and to propose alternative solutions. The article assesses level of employment of the disabled and its dynamics. The factors determining level of employment of citizens with disabilities are systematized. There legible relationship between this indicator and severity of the group is established. The conclusion is made that the most complicated solution concerns issue of employment of the disabled with groups I and II. Very often the problem of employment of such citizens is resolved by social employment but its indicators are low. However, the problems of employment occur and in the group of the less seriously disabled of group III. The conclusion is made that it is necessary to implement comprehensive approach to employment of the disabled. At the same time, it is reasonable to follow differentiated approach depending on the group characterizing severity of disease and type of functional disorder.

THE REGIONAL TRENDS IN NATALITY AND MORBIDITY OF POPULATION: CHARACTERISTICS OF PRE- AND POST-COVID PERIODS

Abstract
The issue of national demographic well-being of population is the most actual task in the national state programs of social economic development and security. The tools implementing these tasks at state level are the current Concept of Demographic Policy of the Russian Federation for the period up to 2025, the Decree of the President of the Russian Federation “On the National Development Goals of the Russian Federation for the period up to 2030 and in perspective up to 2036” and also a number of other normative legal acts. The study of regional characteristics permits to identify specifics of demographic problems and to develop targeted measures considering local conditions and needs of population. The presented results can be used as basis for assessing demographic trends at level of the Regions of Russian Federation and can be considered as development of theoretical propositions in the field of demographic process management at Federal and Regional levels. Besides, the results of analysis and modeling make it possible to assume that demographic processes in the Regions are determined not only by the level of morbidity of population, but by a number of other factors of social, economic and institutional nature that can be assumed as basis for subsequent studies of this issue.
