Phase Dynamics of the Covid-19 Pandemic. A Systematic Analysis of 213 Countries and Territories. Report 2

Abstract
The study was carried out to evaluate the dynamics of monthly numbers of cases, deaths, tests and case fatality ratio worldwide during three phases of the COVID-19 pandemic.Material and methods: Twenty-three sets of databases, dated the 22nd of each month from January 2020 to November 2021, for 213 countries were collected from the Worldometer website. The number of cases, deaths, tests, case fatality ratio, infection fatality ratio, etc. were counted for various periods of time for each of the 213 countries, then the results related to different periods of time were compared.The analysis of main epidemiological parameters resulted in division of three phases of the global pandemic evolution. The first phase (23.01.20-22.07.20), the second phase (23.07.20-22.01.21) and the third phase (23.01.21-22.07.21) were different in terms of the number of tests performed, new cases and mortality due to COVID-19. By the end of second phase, the worldwide statistics indicated end of the pandemic, but the third phase was characterized by sudden rise in number of new cases and deaths. The most dramatic evolution of epidemic curve occurred in the countries where physicians had successfully confronted COVID-19 during the first two phases of the pandemic. Despite the decrease in the overall numbers deaths during the latest months analyzed, additional study is necessary to identify causes of new cases and deaths during the third phase of the pandemic. It can be suggested that preventive and therapeutic protocols should be changed from the 'standard' to 'personalized' types.

The activity of noncommercial organizations providing medical social care to migrants in conditions of COVID-19

Abstract
The article considers actual topic concerning acquisition by migrant associations of certain importance in the public sphere of different countries. The analysis demonstrated that these public structures begin to act as actors of intersectorial cooperation within the framework of state policy in the fields of migration and health care. It is substantiated that various institutions of civil society remained in shadow of state and commercial structures for a long time. Only recently, Russian socially oriented noncommercial organizations acquired status of suppliers of social services, including medical social care of population. At that, they became a subject of cooperation in order to provide medical social care to population, which is provided not only to Russian citizens within the framework of mandatory medical insurance, but also to migrants. It is proved that during the pandemic, many noncommercial organizations expanded scope of cooperation with government, business and public structures of medical and social profile. Some noncommercial organizations that were originally organized as associations of migrants and had character of diasporas are now legitimate NGOs. Such organizational and functional changes can be interpreted as a result of development in the field of care of migrants and their integration into the Russian noncommercial sector. As a consequence of transnational commitment of migrants themselves and their interest in development of their native countries. Also, as a result of institutional integration, and consequence of spreading of COVID-19 and intensification of need in medical and social care to countrymen and their families. It is concluded that institutional changes allow NGOs to establish more open relations with associations of migrants of various origins, as well as to attract additional resources for planning, financing and implementing projects that are strategically important both for survival of organizations themselves and for helping migrants in pandemic conditions.

The Russian demography in 2010-2020: scientometric analysis according Elsevier data

Abstract
The actuality of demographic research is permanently at high level both in Russia and in the world. The purpose of the study is to establish placement of studies and researchers in Russia on the world map of demographic science. The scientometric analysis was applied to the base of international quoting Scopus (Elsevier). The analysis of universe of discourse “Demography” was implemented using SciVal (Elsevier) on the basis of number of publications, number of viewing abstracts or full texts of publications, number of quoting, weighted-average impact-factor of journals. To compare trends according number of publications and quoting in the universe of discourse eLibrary data was used. The results of analysis demonstrated significant lagging of Russian publication activity on demography from countries-leaders on scientometric indices. At that, increased interest to Russian scientific results was established at the level of publications viewing. The most notable place in research of prominent topics in demography the Russian researchers assign to problematics of natality and inner migration. The number of directions targeting to ameliorate considered situation and to place the results of Russian demographic studies on world map of demographic science was proposed.

The gender and age differences of persons with suicidal behavior

Abstract
The study examines cohort of people with suicidal behavior in the light of gender and age differences. The archive materials of the Kaluga Regional Psychiatric Hospital for the period of 2017-2018 were studied. The allocated 435 cases of hospitalizations with suicidal behavior are allocated to admissions with suicidal ideation and hospitalizations with suicidal attempt and self-harm with specified suicidal intentions. The data obtained was processed using the Microsoft Excel software applying the Yule's coefficient of association and Pearson's contingency coefficient, Pearson's χ2 test and Student's t-test. The results of study established no differences in suicidal manifestations adjusted for gender, but allow to distinguish risk age intervals. Thus, mean age in the group of admission with suicidal ideation was in the range of 29.8-35.2 years and 36.0-41.4 years in the group of suicidal attempt. In the age cohort of 5-19 years, probability of admission with suicidal ideation was higher than with suicidal attempt and self-harm. On the contrary, in the age range of 25-44 years, admission with suicidal attempt and self-harm prevailed. This information complements scientific evidence and points out directions for further research concerning prevention of mortality from external causes (suicide) in this area of public health theory.

The organizational legal forms of noncommercial organizations in health care of The Russian Federation

Abstract
One of the goals of development of health care system is to increase availability and quality of medical care of population. To achieve this goal is possible by continuous improvement of health care system, search of new solutions to assigned tasks, development of medical care of population. Over the past 20 years, health care system of the Russian Federation underwent a number of significant changes. In the Strategy of Development of Health Care of the Russian Federation for the Long-Term Period 2015-2030 it is noted that to achieve highest possible level of development of health care with coordinated application of all national resources it is necessary to form health care system that integrates all medical services and organizations, regardless of their form of ownership and affiliation. This system is to function within single regulatory and legal framework (uniform requirements for quality and availability of medical care, qualifications of medical personnel, etc.), as well as uniform state, expert and public control. In this regard the study of prospects of using in health care of various organizational and legal forms of noncommercial organizations that contribute to formation of competitive environment in the industry, is of particular relevance, which should also affect quality of medical care provided.The article considers prospects for development of those organizational and legal forms of noncommercial organizations that are already functioning in health care system: state budgetary health care institutions, state autonomous health care institutions, autonomous noncommercial organizations, private institutions.According to the Federal Foundation of Mandatory Medical Insurance Register of medical organizations, on June 2021 about half of medical organizations in the Russian Federation are registered as budgetary organizations. Accordingly, predominant organizational and legal form is budgetary institution, which makes up to 55% of all medical organizations of various organizational and legal forms involved into mandatory medical insurance system. The percentage of state autonomous institutions comes to 8%, private healthcare institutions - 2%. The percentage of autonomous noncommercial organizations comes to 0.4%. The perspectives of development of each of these organizational and legal forms in health care system are analyzed in the article.

The “digital pathogen” concept: epidemiological approach to problem of disorders associated with information technologies

Abstract
The article is devoted to conceptualization of current scientific experience of studying disorders associated with information technologies. Over the past decades, prevalence of information technologies in world modern society occurs with such intensity that detailed study of influence of this factor on health is significantly lagged behind. In the last decade, a number of pathological conditions associated with application of modern information technologies, in particular psychological and psychiatric profile, was described. The new concepts of Internet addiction, gambling, problematic use of technologies, etc., describing individual response of patients, are introduced. At that, lack of fundamental understanding of both etiology and pathogenesis of described conditions due to multiform specifics, high comorbidity and active evolution of digital society.The article attempts to fill this gap and to approach issue on population scale, namely to evaluate from perspective of epidemiological method characteristics of occurrence, spread and termination of pathogenic effects. The analysis of existing studies suggests that causation of emerging disorders is related not only to addictive behavior and prolonged exposure to information flow, but also to different pathogenicity of its particular components, causing various clinical forms of health disorders in individual with different levels of susceptibility. The pattern of new unique pathogen with specific propagation in human population is described - indirectly through digital environment - and capable of replication within specific ecological niche, i.e. digital pathogen tropic to bio-substrates of higher nervous activity. At that, the duration of contact with digital environment is considered as necessary but insufficient cause of development of pathological process that corresponds to empirical data. The most important property of digital pathogen propagation is its ability to replicate in anthropogenic abiotic digital environment, which is conditioned by its nature, mechanisms of accumulation and distribution.It is also noteworthy that basic principles of dissemination of information flows in social networks largely repeat principles of propagation of infectious pathogens in nature and are characterized by unique mechanism and ways of its transmission.It is concluded that in modern conditions there is a need to study problem of pathology associated with digital environment at population level from position of epidemiological method. The important aspects are joint efforts in studying and classifying new pathogen at interdisciplinary level and developing preventive measures.

The analysis of attitude of patients to rendering of paid medical services in municipal polyclinic

Abstract
In modern society conditions, particular importance is attached to implementation of concept of relationship marketing in organization of provision of medical services. At that, implementation of marketing approaches in interaction between physician and patient, as well as between patient and administrators of department of paid medical services as primary link of contact “patient - medical institution” is of great importance.Purpose of the study is to develop portrait of consumer of paid medical services in state budgetary medical institution.The method of continuous questioning was implemented in study during 2015-2018. The sampling included 343 respondents.The survey demonstrated that main consumers of paid medical services in Moscow budgetary medical institutions are individuals of able-bodied age, married and with income per family member from 10 to 30 thousand rubles per month. Most of respondents are satisfied with quality of paid medical services. According to survey results concerning patients preferences of various forms of participation in paying for medical services, official payment to cashier was the most preferable form (45% of respondents), 73% of respondents noted their positive attitude towards paid medical services in municipal polyclinics and 61% noted that availability of paid medical services in state budgetary medical institutions helps to more fully implement right of citizen to medical care, provided that they are not included in mandatory medical insurance program.The high level of patient satisfaction with provision of paid medical services was revealed for following aspects: competence and courtesy of physician, convenient location of medical institution. There is positive attitude of patients to availability of paid medical services that are not included in state program in territorial polyclinic, related to increasing of territorial accessibility when receiving them at relatively low cost.

The estimation of quality of life of children with diseases of musculoskeletal system

Abstract
The study of life quality was carried out covering 432 children aged 6-17 years with diseases of musculoskeletal system on the basis of the cardio-rheumatological department of the Republican Children Clinical Hospital and children polyclinics of the Republic of North Ossetia-Alania. The Russian version of the SF-36 questionnaire was applied which included 11 questions. The physical and emotional state was studied in 4 age groups: 6-8 years old (8.56%), 9-11 years old (20.14%), 12-14 years old (35.65%), 15-17 years old (35. 65%). The average age of children was 13.06±3.9 years. The significant influence of rheumatic diseases on parameters of life-quality of patients was established: physical and psychological state of children suffers, social adaptation is disturbed. According to the study results, the most problematic age group are children aged 15-17 years, who characterize their condition as mediocre (35.1%). Particular attention should be paid to the fact that there was no improvement in health state during a year among significant proportion of patients (65.3%). The obtained results testify limitation of physical state of health in 78.5% of cases, the most noticeable in age group of 15-17 years. The direct correlation between increase in pain syndrome and age of patient is established. In all age groups, there is decrease in emotional state, which increases with age. The indices of physical component of health in all age groups are significantly lower than the mental one. There are no significant gender differences in respondents assessment of life quality. The indices of life quality of children are generally characterized as low and do not reach 50 points on 100-point scale.The identified problematic age groups require increased attention from pediatric service of the Republic. The anxiety is caused by increase of negative symptoms in older age groups. The study results indicates both low efficiency of specialized rheumatological care of children and need for increased attention to dispensary work with this group of patients at early stages of disease.

The characteristics of physical development of present-day students

Abstract
The problem of preserving health of students is of great public importance. In assessing state of health and morphofunctional state of human body, the most important aspect is level of physical development. The purpose of the study is to investigate indices of physical development of students of medical university and to implement their comparative evaluation. The assessment was carried out concerning physical development of students of the Pacific State Medical University (238 girls and 57 boys) in 2020, including anthropometric measurements, calculation of BMI and Pignet index. The factor analysis was performed. It was established that anthropometric indices of male and female students had statistically significant differences. The BMI assessment of students demonstrated that overweight and obesity more often observed in boys, as underweight in girls (p=0,026). The analysis of strength index of students physique established that insufficient development of skeletal muscles, requiring correction, was detected in 58,4% of girls and 33,3% of boys. The factor analysis of complex of factors determined that most completely explained relationship between criteria for physical development of students (p<0,001).

The abortions in minors as a medical social problem

Abstract
The article deals with the problem of abortions among minors, which has extremely important socio-demographic significance and affects interests of the state, society, family and, above all, interests of young woman herself. The purpose of the study is to investigate and to analyze situation with abortions among minors in the Russian Federation. It is noted that maximal prevention of abortions among minors is a task of particular importance, requiring extensive educational and explanatory measures, improving the life-style, key direction in health protection strategy not only of adolescents and the youth, but also of population as a whole.The study identified the range of problems young mothers face. The conclusion is made that program to support them should be targeted to both minors themselves, their families, sphere of education, medicine, social protection of population and mass media. At the same time, social, economic, legal, medical, psychological and pedagogical consequences of these phenomena require significant state financial expenditures to eliminate them.The study substantiated that only integrated approach and application of modern means of education and medical prevention in solving problem of sexual education and education of adolescents will allow to obtain positive results. And if it is successfully implemented, it will result in strengthening health of entire nation of the Republic, significantly reducing increased threat of sexually transmitted diseases, early abortions, young motherhood, etc.

The medical demographic characteristic of group of citizen injured in road accidents on the Federal Highway

Abstract
The role of citizen of elderly age in road accidents on the Federal Highway was principally passive since passengers of vehicles predominated (p<0,0001). The most of victims of elderly age (66,7%) suffered of combined lesions. Considering gender of victim, presence of ethanol in blood at the moment of hospitalization, the role of victim of road accident and degree of severity of lesions in citizen of elderly age was in overall higher than in citizen of able-bodied age up to 2.3-2.9 points according ISS scale. The established medical demographic characteristics of victims of elderly age condition necessity of development of differentiated approaches to their medical care support, planning of measures of population prevention of medical sanitary aftermaths of road traumatism.

The problems of security of officials of customs agencies under carrying out customs inspection of containers treated by fumigants

Abstract
The use of various harmful substances for fumigation and preservation of goods during their transportation in containers to preserve it for longer time are widely used in sea transportations. In the context of increasing demand for carriage of goods in containers by sea, issue of customs officials safety, who are, as part of their duty customs inspection, the first to open container treated with fumigants, is becoming increasingly relevant. The results of analysis established absence of clear legal regulation of issues related to ensuring security of customs officials authorized to conduct customs inspections, both at international and national levels. In this regard, detailed analysis of main fumigants that customs inspector may encounter in his professional work is presented in this article. The risks of negative effect of fumigants on inspector health are studied and appropriate conclusions and recommendations regarding the legal and technical regulation of this question are formulated.

The effect of green stands on human health

Abstract
The article describes the role of green stands in preserving and improving population health. They positively effect human emotional background, improving cognitive functions and mood, forming subjective feeling of cheerfulness, restoring vitality and contribute to normalization of somatic health. The article presents criteria for assessing quality of green stands. The recommendations on planning and designing green stands, considering climatic requirements are given. It is concluded that urban trees are a significant factor that brings economic dividends to population.

The specifics of interaction with patients and their relatives as a factor of emotional condition of oncologists

Abstract
The paper considers issue of professional burnout of oncologists through the prism of their relationship with patients and their close relatives. The study was carried out using direct and online sociological survey method. The sample included 534 specialists being in constant contact with cancer patients. The results revealed pronounced prevalence of such main burnout domains as emotional exhaustion (55%), depersonalization (19%), reduction of professional achievements (22%). The most common methods of dealing with stress were recreation, sports, communication with immediate relatives, hobbies, walking (7-20%), 4% used alcohol. The high frequency of patient requests to oncologist for psychological support (88%) was established as well as difficulties in communication between doctor and patient because available communication skills are not always sufficient for 76% of oncologists. The majority of respondents support idea of patient need for psychological support and its effectiveness for their treatment and physical and emotional conditions. The similar trends were found in case of close relatives of patients. At the same time some oncologists expressed negative, stigmatizing attitudes towards patients in need of psychological support that requires additional educational work. In oncologists, even when need for psychological support is expressed by patient, willingness to seek it is low: a third of respondents are “rather not ready” to see psychologist even if they had this opportunity. The results meet data of other researches data and reveal that interaction with patients and their relatives is one of the main factors affecting the emotional state of oncologists. Based on study results as well as on publication analysis, comprehensive solution was proposed to implement psychological oncologic service in Russia, which, on one hand, will allow to provide support to cancer patients and their relatives, and on the other hand, will conduct specific training for oncologists concerning skills of psychologically safe communication with patients and coping with negative experiences.

The health-preserving culture of family of Russian serviceman

Abstract
The article considers specifics of health-preserving culture of family members of serviceman and parameters of its sociological assessment. It is proved that military personnel consider family as social element of military collective and at the same time as sociocultural subsystem of military unit. The latter determines subordination of sociocultural interests of serviceman family to interests of development of military organization, which manifests itself as a factor complicating development, strengthening contradictions in system of health-preserving culture of this family. It is proved that in modern conditions it is necessary to strengthen state regulation of processes of formation of health-preserving culture of family of serviceman by inclusion in system of existing programs of physical culture and sports of military units of entire serviceman family, activation of interaction of military health experts in order to develop knowledge, formation of specific norms, rules of health-preserving behavior of family of military personnel; by provision of organizational, sociocultural, economic conditions for self-development of culture of health-preserving behavior of family members of serviceman; by activation of external social control, expansion and improvement of effectiveness of army social services, prevention and suppression of destructive health behavioral practices not only of serviceman, but also of his family members.

The healthy life-style in the system of value orientations of student youth

Abstract
The article is devoted to the issue of studying attitude of students to their own health, to healthy lifestyle, to regional health care system. The results of sociological survey carried out in February-April 2021 are presented. The factors and conditions contributing to preservation and strengthening of youth health were identified. The conclusions and recommendations concerning formation of value orientations for healthy lifestyle of students were formulated.

Certain aspects of management of health preservation system

Abstract
The trends in development of National health care in Russia are considered. The need in formation of intersectorial system of management of health protection activities is established with priority of preventive direction, including mental prevention, based on systematic observation and participation of both sick and healthy people. The system goal was determined - to improve population health. The main tasks and principles of system functioning are formulated.The reasonableness of application of mechanisms of motivating and stimulating all subjects of health protection system in order to increase their interest, is indicated as one of directions of organizing and introducing intersectorial health protection system. The basic principles of development and implementation of incentive systems of remuneration were formulated.The attention is focused on such aspect related to management of health protection system as legal support. The review of sources of law in the field of health protection revealed that all subjects have duties, and everyone has responsibilities, except citizens and insured persons. To resolve this problem, it was proposed to conclude agreement between the parties, indicating rights and obligations of both parties and responsibility for violation of contractual obligations.The structure is proposed, the main principles of health protection system management are described, designed to provide real shift in achieving settled goals.

The information communication technologies in medicine

Abstract
The article discusses some aspects of information and communication technologies in medicine. In view of the COVID-19 pandemic, special attention is paid to development of telemedicine. The informational computer technologies are especially relevant where there is shortage of medical personnel. Taking into account that by the year 2035 there will be shortage of 12.9 million medical workers worldwide due to aging of population, increase in prevalence of non-communicable diseases, migration and turnover of medical personnel, remote consultations of patients are becoming increasingly relevant. The examples of information and communication technologies in health care that proved their effectiveness are given.

The development of organization of population medical care in conditions of digitization of public health

Abstract
The article discusses two main problems of organizing medical care in national health care of Russia: medical and social monitoring of morbidity and mortality; key performance indices of public health in the Russian Federation. Methodologically, the article is based on interdisciplinary scientific approach that allows to study medical, social and economic components of public health, considering observed trends: digitalization of health care and epidemic of new coronavirus infection. Based on statistical analysis of open data on morbidity and mortality of population in the Russian Federation over past few years, results were obtained indicating that collection of medical statistics is not built consistently that medical care in health care is not organized sequentially as cycle or process, but is presented in fragments. This leads to the fact that rural population has very limited access to multidisciplinary and high-tech medical services, and in structure of mortality there is high gender bias (males die more often than females in same classes of causes). Based on identified problems, two organizing solutions are proposed. The first solution is to create, on the basis of the Ministry of Health Care of Russia, special portal for complete, reliable and up-to-date medical statistics, which will be generated and processed using Big Data technologies. The second solution consists in transition from fragmentation to systematic organization of medical care in the “education and prevention-treatment-rehabilitation” continuum, while for each stage special set of key performance indices is established that allows to solve the tasks of improving public health, but also to develop harmonized social and economic public policy.

The calling a doctor in: what an optimal model can be? (publications review)

Abstract
The primary health care represent the most common segment of medical care in many countries and provides prevention and treatment of diseases, dynamic monitoring of patients with chronic diseases and medical rehabilitation. There is substantial shortage of health personnel to meet health needs all over the world, especially in primary health care. The COVID-19 pandemic exacerbated this problem. It changed workload in primary health care and demonstrated need for new organizational solutions to increase access to primary health care.One of the new transformations in general practice can be redistribution of medical work in primary health care. In this research, five options for organizational models of primary home care were identified. We used “performer (doctor / nurse)”, “payment method for the home visit”, “frequency of patient visits per week” as indices.There is lack of unified approach in the way how home visits are organized so optimal organizational model of primary home medical care is needed. This will increase access of most demanded type of medical care and ensure rational use of human resources at the primary health care level, especially during COVID-19 pandemic.

Abstract
The patients with aneurysm subarachnoid hemorrhage (aSAH) living in remote, inaccessible, rural areas cannot be provided with urgent neurosurgical care on the spot. They require medical evacuation (ME) to neurosurgical hospital. The purpose of the study was to investigate effect of complex (multi-stage) and simple (one-stage) logistic scheme of ME of patients in acute period of aSAH on the outcome of the disease. The retrospective analysis of results of surgical treatment in 145 patients with aSAH hospitalized in regional vascular center (RVC) in Yakutsk in 2017-2018 was carried out. The subjects were allocated into 3 groups: group 1 - patients from regions of the Republic of Sakha, who underwent ME to the RSC using simple transportation scheme; group 2 - patients from regions of the Republic of Sakha who underwent ME using complex transportation scheme; group 3 (control) - patients hospitalized from territory of Yakutsk.The RVC hospitalized 145 patients. The sanitary aviation delivered 91 (62.8%) patients from districts of the Republic to the RVC. The duration of time from the onset of disease to surgical treatment: in group 1-2 days; in group 2-4 days; in group 3, 2 days (p = 0.018). The frequency of re-rupture of cerebral aneurysm in the group 1 and group 2 did had no statistically significant differences (19,1% and 32.7%) (p = 0.142). Mortality: in group 1 7.1%; in group 2 8.2%; in group 3 7.4% and no statistically significant differences between groups 1 and 2 (p = 1,000), between groups 1 and 3 (p = 1,000) and between groups 2 and 3 (p = 0.886). When applying complex logistic scheme of ME, transportation over considerable distance does not deteriorated course of disease and results of surgical treatment of patients with aSAH in acute period of hemorrhage.

The implementation of cold chain approach at transportation and storage of immunobiological preparations

Abstract
In the modern conditions of the coronavirus pandemic, the issue of transportation of immunobiological drugs is particularly actual, since control measures in this direction provide for supply of vaccines to different states, that naturally involves duration of transportation and compliance with certain established temperature regime at all stages of logistics chain, violation of which results in loss of pharmaceutical properties by drugs. The analysis demonstrated that currently transportation of immunobiological drugs is carried out within the framework of cold chain approach, which establishes mandatory compliance with temperature range of storage and transportation. Thus, the most important issue is control of compliance with temperature regime, which requires appropriate technical equipment at all levels of cold chain, ensuring fixation of detected violations.

The health preserving technologies in process of teaching children with health limited capacities

Abstract
The article discusses health saving technologies in context of education resources for children with disabilities. The purpose of the study is to identify and to classify main groups of health preserving technologies applied in educational practice for children with disabilities. The need to analyze national and foreign studies, concerning hygienic, physical culture, health improving, environmental, educational health preserving technologies targeted to ensure safety of children, is emphasized. The characteristics of their impact are systematized and role of each group of health preserving technologies in optimization of educational process are determined. The recommendations for their application in mass practice are formulated. The scientific novelty of the study lies in systematization of main groups of health preserving technologies that are effective in organizing educational process of children with disabilities.The main results and conclusions of the study are essential characteristics of health preserving technologies in context of resource provision of educational process, as well as effects of their application in educational process for children with disabilities. The specific input of the study of problem is comprehensive analysis of health preserving technologies as special resource to be applied to actualize wide range of components of resource support of educational process for children with disabilities.

The medical activity and satisfaction of geriatric care of Mongolia residents

Abstract
Nowadays, process of aging of population covers all countries of the world. In current conditions, an important moment is increasing medical activity of elderly population and old persons. The purpose of the study is to investigate social hygienic characteristics of medical activity and satisfaction with geriatric care of the elderly residents of Mongolia.The sociological survey of 726 residents of Mongolia older than able-bodied age, 75 out of which were in nursing homes, was carried out using specially designed questionnaire that included questions on assessing medical activity and satisfaction with geriatric care.The negative correlation was established between good assessment of one's own health and age of respondents. The study revealed that 90.9% of respondents noted presence of chronic pathology, but only 21.1% of them were registered in dispensary. In age group under 65 years, 75.8% of persons capable to serving themselves made up 75.8% and in age group 75 years and older only 48.1%. The analysis established that 50.2% of elderly and senile persons consulted geriatrician, while 48.3% of those who did not consult this specialist were unaware of its existence. The number of cases of respondents visiting geriatric doctor in place of their residence was higher in cities. The respondents who received geriatric medical care were completely satisfied with it in 52.7%. The causes of dissatisfaction with geriatric medical care in aimags (regions) were inattentive attitude of medical staff, low effectiveness of treatment and insufficient availability of free provision of medications.The identified social hygienic characteristics of medical activity and satisfaction with geriatric care of the elderly and senile residents of Mongolia should be taken into account in developing measures to improve this type of care in Mongolia.

The implemented historical reconstruction was based on materials of section of railroad medicine of Congresses of the Society of Russian Physicians in memory of N. I. Pirogov. The issues are analyzed concerning legal basis of railroad medicine, duties of services and position of railroad physicians, principles of organization of medical sanitary unit and medical care of passengers, organization of sanitary supervision of status of railroad workers and ensuring safety on railroad transport.

The history of becoming of syphilitic hospitals at the North-Eastern outlying districts of Eastern Siberia in XIX century

Abstract
In the XIX century, process of joining of the North-East lands to the Russian empire was completed. The syphilis that spread in XVIII-XIX centuries stroke indigenous population of the Eastern Siberia and out-migrants from other territories. The article attempts to highlight little-studied aspects of organization of syphilitic hospitals and medical care of newly come population and non-Russian communities in northern Okrugs of Yakutsk, Kamchatka and Primorye areas of Eastern Siberia. On the basis of archive materials, the investigation was applied to the most important components of organization of medical care of patients with syphilis: the organization of syphilitic hospitals, history of correspondence and decision-making on funding sources, building of yurts, choice of buildings for hospitals, delivery of medications, invitation and appointment of physicians, medical assistants and healers to assist population with medical care. The role of the Irkutsk Prikaz of Public Charity and the Irkutsk Medical Board in this process is noted. The establishment of syphilitic hospitals in outlying territories of Yakutia, Kamchatka and Primorye was the most important onset of struggle with syphilis epidemic in population.

“The letters of S. P. Botkin from Bulgaria. 1887”: The return of the manuscript in Russia

Abstract
The previous report partially brought to light history of publishing of “The letters of S. P. Botkin from Bulgaria 1877”. The original letters were abroad for many years and they were returned to the USSR in 1974 by grandchildren of S. P. Botkin, S. P. Chekhov and D. P. Chekhov, who resided in France. The circumstances of handing over of the letters to the N. A. Semashko Institute of Health Organization and Social Hygiene are presented. The letters have not been republished for 130 years, but their historical, moral and educational significance remains actual and nowadays. The modern edition of the complete manuscript, along with introductory article and detailed scientific commentary, will become a significant contribution to representation of biography of S.P Botkin, to supplementation of written sources about the Russian-Turkish war of 1877-1878, and to comprehension of centuries-old Middle Eastern geopolitics of Russia.
